Job summary

We are recruiting a Urgent Care Practitioner to support our experienced Clinical team.

Primary Care experience is essential.

Main duties of the job

Assess and triage patients, including same day triage and as appropriate, provide definitive treatment (including prescribing medications following policy, patient group directives, NICE or make necessary referrals to other members of the primary care team.

About us

We are a large supportive training practice based in Telford with 24,800 patients and based over two sites. We have a large clinical team consisting of 16 GPs and30 clinical staff, including nurses and allied health professional all supported by the administrative team.

Job description

Job responsibilities

Assess patients presenting with acute illness or for review of acute exacerbations of chronic disease conditions.

Undertake home visits with an elderly care home setting for acute illness.

Diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.

Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.

Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate.

Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug based treatment methods into a management plan.
