Job summary

Act as the first point of contact for patient queries working in the triaging hub, managing minor ailments, acute medication requests, Structured medication reviews & QOF work.

Main duties of the job

  • Acute Triage and Minor Ailments:Assessing, diagnosing, and managing minor illnesses, including telephone triage and providing advice, or signposting to appropriate services (e.g., community pharmacy).
  • Clinical Medication Reviews:Conducting structured medication reviews (SMRs) for complex, long-term conditions (e.g., hypertension, diabetes) and managing polypharmacy.
  • Prescription Management:Handling acute prescription requests, repeat prescription reviews, and managing medication changes for patients recently discharged from hospital.

Qualifications

Essential

  • Pharmacy degree and registration with the General Pharmaceutical Council (GPhC).

Desirable

  • Prescribing Pharmacist.

  • Ideally at least 2 years post-graduate experience in general practice.
