Job summary

Dr Gellia & Dr Balaraman's Practice is based in Monkwearmouth Health Centre and we have an exciting opportunity for a registered Practice Nurse to join our friendly team. We currently have a population of around 4700 patients and we are looking for an experienced nurse to work alongside our existing practice team in order to meet the needs of the patient and liaise with other members of the primary healthcare team.

Main duties of the job

Registered General Nurse, experience preferred in chronic disease management including: Asthma, COPD, Coronary Heart Disease, Hypertension and Diabetes.

Other duties include:

* Adult and childhood immunisations

* Travel vaccines

* Infection Control

* Ear Syringing

* Cervical Screening

* Contraception and sexual health advice

About us

We are a friendly and welcoming, well established practice with 4700 patients with a Good CQC rating. The Partnership comprises, Drs Gellia and Balaraman.

We currently employ 3 part-time Practice Nurses, 1 Advanced Nurse Practitioner and 1 Health Care Assistant and are supported by a full team of Administrators. We also work closely with many Primary Care Network roles including Pharmacists and Mental Health Practitioners.

Job responsibilities

The following are core responsibilities of the Practice Nurse role, the list is not exhaustive and other tasks may be required within the remit of the role, practice and patient needs. The role includes:

To be responsible for the delivery of practice nursing services, delivering care within their scope of practice to our patient population.

To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice.

To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing in line with national and local policies and practice initiatives and referring patients to secondary/specialist care as required.

To review patient medications, in line with long term condition review, to enhance compliance.

To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health.

To maintain chronic disease registers and support the Practice Manager with QOF.

To implement vaccination programmes for adults and children in line with policy and national guidance.

To request pathology services and process pathology results as required and manage and maintain cervical screening results and recalls.

To maintain accurate medical records, including the use of appropriate templates, and ensure SNOMED codes are used effectively ensuring compliance with practice recall system.

To develop, implement, embed and participate in NHS Health Checks.
