Job summary

We are recruiting for an Advanced Nurse Practitioner to join our team. The successful candidate will be responsible for the delivery of nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The Advanced Nurse Practitioner will be responsible for a number of clinical areas such as heath promotion, chronic disease management, health prevention, well women and well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.

Main duties of the job

The following are the core responsibilities of the nurse practitioner:

a. Develop, implement and embed health promotion and well-being programmes

b. Provide a triage service, actively managing or referring patients or treating patients within own scope of practice

c. Support the clinical team in the development of individual treatment plans for chronic disease patients & support patient at risk of developing long-term conditions

d. Provide wound care (ulcer /Doppler etc.) and travel medicine services to patients

e. Request pathology services as necessary, processing and interpreting results as required

f. Maintain accurate clinical records in conjunction with extant legislation

g. Develop, implement and embed well woman & well man clinics

h. Support the team in dealing with clinical emergencies

i. Recognise, assess and refer patients presenting with mental health needs and/orwith community health needs, developing strategies for effective care pathways

j. Implement vaccination programmes for adults and children

k. Support the clinical team with all safeguarding matters, in accordance with local and national policies

l. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

m. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required

Job description

Job responsibilities

The following are the core responsibilities of the nurse practitioner. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. Develop, implement and embed health promotion and well-being programmes

g. Provide travel medicine services

h. Request pathology services as necessary, processing and interpreting results as required

i. Refer patients directly to other services as appropriate

j. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required

k. Maintain accurate clinical records in conjunction with extant legislation

l. Ensure read codes are used effectively

m. Maintain chronic disease registers

n. Develop, implement and embed well woman clinics

o. Develop, implement and embed well man clinics

p. Chaperone patients where necessary

q. Assist GPs with minor surgery when required

r. Prioritise health issues and intervene appropriately

s. Support the team in dealing with clinical emergencies

t. Recognise, assess and refer patients presenting with mental health needs

u. Recognise, assess and refer patients with community health needs, developing strategies for effective care pathways

v. Implement vaccination programmes for adults and children

w. Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

x. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)

y. Support the clinical team with all safeguarding matters, in accordance with local and national policies

z. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

aa. Deliver opportunistic health promotion where appropriate

In addition to the primary responsibilities, the nurse practitioner may be requested to:

a. If an independent prescriber, prescribe in a safe an effective manner, in accordance with PGD and extant legislation

b. Act as the audit lead, effectively utilising the audit cycle

c. Monitor and ensure the safe storage, rotation and disposal of medicaments

d. Contribute to the implementation of an effective training programme for staff and students within the practice

e. Provide clinical supervision for the nursing team

f. Participate in local initiatives to enhance service delivery and patient care

g. Support and participate in shared learning within the practice

h. Continually review clinical practices, responding to national policies and initiatives where appropriate

i. Be aware of and contribute to the planning and implementation of CCG patient pathway initiatives

j. Participate in the review of significant and near-miss events applying a structured approach i.e. root cause analysis (RCA).

k. Maintain professional registration and adhere to revalidation requirements
