Job summary
We are looking to recruit a hard working and enthusiastic pharmacist to join our busy multi-disciplinary team.
The role is pivotal to improving operational efficiencies and the quality of care so we are looking for a candidate who is motivated and passionate about delivering an excellent service within general practice.
The successful candidate would be responsible for implementing an effective medicine management system, including cost effective prescribing, so would work as an independent prescriber (or would be supported to work towards independent prescribing status if the qualification is not already held).
We are looking for the post holder to work 22.5 - 27 hours per week, across 3 days. We are wanting to recruit into a Thursday and Friday, along with another day of the post holders choosing.
Main duties of the job
The post holder will:
- Perform medication reviews for patients with polypharmacy - especially for those with frailty and/or with multiple co-morbidities and will maintain the medication review system
- Provide medicines reconciliation following discharge from hospital, including reviewing discharge information and updating the patient's repeat medication list as necessary
- Monitor and review high risk medicines and shared care prescribing
- Provide expertise in medicines advice and support general practice staff with regards to prescription and medication queries
- Take responsibility for areas of chronic disease management that falls within competency
- Monitor the use of special drugs and give advice regarding alternatives to unlicensed medicines
- Liaise with local community pharmacists and act as a point of contact regarding any prescription queries, amendments or appropriate switches for out of stock medicines
- Proactively provide prescribing advice to prescribers and clinical staff regarding changes in drug safety information, discontinued products and changes in licensing.
- To contribute to quality improvement and clinical audits, as well as supporting aspects of the Quality and Outcomes Framework and antibiotics stewardship.
- Ensure prescribing in the practice is evidence based and follows local and national guidance.
