We are looking for an experienced Diabetes Nurse to join our large but friendly GP Practice in Westbury, Wiltshire.

The successful applicant should be a registered nurse, ideally with a diploma or extensive experience in Diabetes management.  Candidates should be motivated to support patients to live well with diabetes and ensure they receive the best care possible. 

Main duties of the job

You will be required to undertake nurse led clinics to assess, diagnose, plan, implement and evaluate treatment/interventions for patients presenting with diabetes, pre-diabetes or undifferentiated diagnosis.

Using your knowledge, within scope of competence, to deliver holistic care to people accessing primary healthcare services.

PLACE OF WORK: Westbury Group Practice

HOURS OF WORK: Monday – Friday between 08:00am and 20:00pm and Saturday 9am-5pm

Job summary:

The post holder is responsible for the delivery of diabetes nursing services to the practice population. The post holder will be supported by the Clinical Lead Nurse and GP Diabetes Lead within the practice. They will work collaboratively within the multi-disciplinary team to meet the needs of patients, support the delivery of policy and procedures and improve our service delivery.

They will demonstrate safe, clinical decision-making and expert care for patients within the general practice.

The post holder with provide expert advice to patients, carers and colleagues to ensure the maintenance of clinical excellence and provide advice in respect of ongoing care needs. The post holder will need to prioritise care needs of the patients accordingly and request investigations in the appropriate manner and make referrals as required.

The post holder will work within NMC Code of Conduct and will be accountable for their own professional actions within professional guidelines.

Main Duties and Job Responsibilities

  • Undertake clinical nursing practice at an advanced level using knowledge, within scope of competence, to deliver holistic care to people accessing primary healthcare services.
  • To operate nurse led clinics to assess, diagnose, plan, implement and evaluate treatment/interventions for patients presenting with diabetes, pre-diabetes or undifferentiated diagnosis.
  • Refer patients directly to other services/agencies as appropriate
  • Providing assessment, screening treatment services and health education advice.
  • Undertake pathological specimens and investigatory procedures

· Assist in and perform routine tasks related to patient care

· Work with other members of the practice team, to plan and deliver diabetes services, promoting integrated and seamless pathways of care.

  • Support and mentorship to other colleagues within the practice who are seeing similar patients.
  • Provide safe, evidence-based, cost-effective, individualised patient care
  • Promote health and well-being through the use of health promotion screening
  • Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery
  • Develop and set up new patient services and participate in initiatives to improve existing patient services
  • Undertake home visits for patients with diabetes as required

Administration and professional responsibilities

  • Participate in the administrative and professional responsibilities of the practice team
  • To maintain comprehensive, accurate and contemporaneous computerised records of all consultations and contacts with patients and when appropriate written or printed records.
  • Ensure QOF and Arden’s templates are completed accurately.
  • Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice
  • Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit
  • Restocking and maintenance of clinical areas and consulting rooms
  • Attend and participate in practice/nursing meetings as required
  • Attend diabetes team meetings
  • Work collaboratively with colleagues within and external to the practice.
  • If it is necessary to expand the role to include additional responsibilities, full training will be given.

Training and personal development:

  • Training requirements will be monitored by yearly appraisal and will be in accordance with practice requirements. Personal development will be encouraged and supported by the Practice. It is the individuals’ responsibility to remain up to date with recent developments in line with NMC Revalidation.
  • Participate in an annual appraisal as per the practices appraisals policy
  • Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of audit and peer review
  • Keep up to date with pertinent health-related policy and work with the practice team to consider the impact and strategies for implementation
  • Participate in the education and training of students of all disciplines and the induction of all members of the practice team where appropriate.
  • If it is necessary to expand the role to include additional responsibilities, full training will be given.
  • The post holder will undertake all mandatory training required for the role.
