North Southwark Primary Care Network (PCN) have an exciting new opportunity for Junior Clinical Pharmacists to join our expanding team supporting our 14 GP practices to deliver high quality, safe and effective services for our population.
In this role you will be an integral part of a multi-disciplinary team (MDT), working alongside our highly skilled clinical pharmacist team contributing to the management of patient care. This will include delivering structured medication reviews and working as a key part of our neighbourhood MDT to optimise medicines use by our registered population. You will have a key role to play in supporting people to manage their long-term conditions.
You will join a growing and mutually supportive team in a forward thinking and stimulating environment. This role will evolve and develop over time and is fundamental to the delivery of high quality primary care services.
Main duties of the job
The ideal candidate will be proactive, enthusiastic and highly motivated. An excellent communicator able to work both independently and as part of a multi-disciplinary team in a diverse, fast moving environment.
You will have passion for working as part of a wider general practice team and directly with people to ensure they get the most benefit from their medicines.
The role is based in North Southwark and travel across the area will be required.

About us
The North Southwark PCN covers a population of circa 190k patients, provided by 14 GP practices, and is supported by Quay Health Solutions (QHS). QHS a community interest company which has, with member practices, developed primary care and integrated services since 2011. This role will be employed by QHS.
The clinical pharmacy team is a highly credible, established team within the PCN that continually evolves to meet the needs of our population, working in partnership with our GP Practices, the CCG and local acute trusts.
Quay Health Solutions does not hold a VISA Sponsorship Licence.
