Job summary
Uppingham Surgery is looking to
recruit an Advanced Clinical Practitioner with excellent clinical skills and
warmth, with a commitment to positively contribute to our provision of high-quality
patient centred care.
The role offers you the opportunity to work as an autonomous
practitioner, working independently to assess, diagnose and treat patients with
a range of acute, non-acute and chronic medical conditions.
Hours: 4 to 8 session practitioner working between Monday and Friday. The position offers a very competitive salary, generous
annual and study leave, with access to the NHS pension scheme.

Develop innovative
concepts, models, methods and practices which deliver new and improved primary
care services to meet the changing needs in Primary Care
Provide education
and training to other staff and students as appropriate and to take a lead role
in encouraging teamwork and sharing best practice
An interest or
skill in an area of long term condition management
Collaborative
working with GPs, ACPs, Clinical Pharmacists, Nursing team and non-clinical
staff to make positive contributions to the organisation and the continued
development of a One Team approach

Job responsibilities
The
Advanced Clinical Practitioner(ACP) has been assessed as competent in
practice using their expert clinical knowledge and skills. They have the
freedom and authority to act and make autonomous decisions in the assessment,
diagnosis and treatment of patients.
The post
holder works at the level of advanced clinical practice and has developed and
can evidence the underpinning competencies applicable to the specialty or
subject area, i.e., the knowledge, skills and behaviours appropriate to the
healthcare professionals (HCP) setting and job role.
The role provides an
opportunity for patients to avoid the need to be referred to a GP, thereby
enabling them to receive timely care and negate unnecessary delay in receiving
treatment.
An ACP is autonomous in making decisions based
on assessment, diagnosis and interpretation of test results and will initiate,
evaluate and modify a range of interventions which may include prescribing
medicines, therapies, lifestyle advice and care.
The
following are the core responsibilities of the Advanced Clinical Practitioner.
There may be, on occasion, a requirement to
carry out other tasks; this will be dependent upon factors such as workload and
staffing levels:
- Practise in compliance with their respective
code of professional conduct and within their scope of practice, being
responsible and accountable for their decisions, actions and omissions at this
level of practice
- Demonstrate a critical understanding of their
broadened level of responsibility and autonomy and the limits of own competence
and professional scope of practice, including when working with complexity,
risk, uncertainty and incomplete information.
- Act on professional judgement about when to
seek help, demonstrating critical reflection
on own practice, self-awareness, emotional intelligence, and openness to change
- Work in partnership with
individuals, families and carers, using a range of assessment methods as
appropriate
- Demonstrate effective communication skills,
supporting people in making decisions, planning care or seeking to make a
positive change
- Use expertise and decision-making skills to
inform clinical reasoning.

- Manage
patients presenting with a range of acute and chronic medical conditions,
providing subject matter expert advice.

and evaluate individual specialised treatment plans for chronic disease
patient.

manage and support patients at risk of developing long-term conditions,
preventing adverse effects to the patients health
- Be a professional role model for the organisation
- Actively engage in peer review to inform own and others
practice, formulating and implementing strategies to act on learning and make
improvements
- Actively seek feedback and involvement from individuals,
families, carers, communities and colleagues in the co-production of service
improvements
- Critically assess and address own learning needs,
negotiating a personal development plan that reflects the breadth of ongoing
professional development across the four pillars of advanced clinical practice
(clinical practice, leadership and management, education and research).

chronic disease clinics, delivering patient care as necessary, referring
patients to secondary/specialist care as required
- Maintain
accurate clinical records in conjunction with extant legislation and to ensure SNOMED CT codes are used effectively
- Develop,
implement and embed an effective call/recall system.
- Critically appraise and synthesise the outcome of
relevant research, evaluation and audit, using the results to underpin own
practice and to inform that of other
- Develop and
implement robust governance systems and
contribute to practice targets (QOF, etc.), complying with local and regional
guidance
- Chaperone
patients where necessary
- Support
the team in dealing with clinical emergencies
- Support
the team with all safeguarding matters in accordance with local and national
policies
