Job summary
Located in the picturesque market town
Uppingham, situated just off the A47, Uppingham Surgery is seeking to recruit a
clinician with a specialist interest in enhanced diabetic care. You will work
in conjunction with our well-established practice nurse and clinical teams.
Hours: 5 hours per week Monday to Friday working in practice or remotely.
Pay: Dependent upon experience
Main duties of the job
What the role involves:
Provide a first point of contact within the practice for the management of diabetic patients
Proactively monitor the care of all diabetic patients in line with local and national policy
Prescribe medication including insulin and manage patients appropriately
Support the clinical team in the development of individual treatment plans for patients with long term conditions
Request pathology services and process pathology results as required
Provide specialist advice and knowledge to newly diagnosed diabetic patients and their families, including dietary advice, and monitoring of their illness, empowering patients to take control of their illness demonstrating empathy, and giving reassurance and support.
Undertake annual diabetic reviews autonomously, commencing/altering treatments as necessary
Deliver patient care as necessary, referring patients to secondary/specialist care as required
Maintain accurate clinical records using ARDENS and SystmOne templates
Act as a resource, providing specialist knowledge, gained through clinical experience and through undertaking diploma level training and expertise to the practice clinical team
