Job summary

We are looking a another member of our team to work as a Practice Nurse with a focus on high-quality specialist care for Respiratory Patients, based on the needs of the Practice population which includes treatment, screening, preventative care and patient education. A key aspects of the role is the ability to be able to perform Spirometry to ARTP standards.

Main duties of the job

  • To work autonomously to deliver specialist clinical care to practice patients with a range of acute and non-acute Asthmatic and COPD conditions.
  • To assess, diagnose and treat Respiratory patients, developing individualised care, management and treatment plans using clinical reasoning. Ability to undertake Spirometry.
  • Implement care plans, including requesting necessary tests, onward referral to other health care professionals and agencies, and follow up consultations where necessary.
  • Ability to undertake Practice Nurse Duties including ECGs, Dressing, General Observations & Phlebotomy

We are a forward thinking Practice, with focus on high quality Patient care and have an appetite for trying new ways of working and employing the use of technology were possible. We have a well established team consisting of Healthcare Support Workers, Nurses, Doctors, Administrative Staff and Paramedics. Job responsibilities

Main Duties and Responsibilities

  • To work autonomously to deliver specialist clinical care to practice patients with a range of acute and non-acute Asthmatic and COPD conditions.
  • To assess, diagnose and treat Respiratory patients, developing individualised care, management and treatment plans using clinical reasoning. Ability to undertake Spirometry.
  • Implement care plans, including requesting necessary tests, onward referral to other health care professionals and agencies, and follow up consultations where necessary.
  • Promote health and screening in relation to primary prevention strategies.
  • Provide advice and emotional support to patients and family or carers for patients living with respiratory conditions, communicating in an empathetic way.
  • Be a clinical resource for patients, and the practice clinical team, supporting developing knowledge and systems to improve health to the practice population and local community.
  • Demonstrate a working knowledge of guidelines for best practice with competency in assessment and treatment of chronic respiratory patients.
  • Participate in the critical evaluation to audit outcomes, or change in patient care practices.
  • Lead by example and action in relation to behaviour, attitudes, conduct and appearance.
  • Maintain accurate patient records, documenting comprehensive data onto the clinical system, SystmOne.
  • Report and manage significant incidents and near misses in line with Practice guidelines.
  • Monitor and maintain stock levels and maintenance of respiratory equipment, ensuring resources are used in an efficient and sustainable manner.
  • Work in close partnership with colleagues and external agencies, to develop care management plans and improve health outcomes.
  • Maintain QOF targets appropriate to the Patients seen.
  • Contribute to the preparation of any Practice development plans. Work towards further improvement and development of patient services recommending areas for change.
  • Actively participate in the delivery of training for staff in Respiratory care contributing to their professional and personal development.
  • Complete all mandatory online training relevant for the role.
  • Work safely always in accordance with Legislative requirements and Practice Policy and Procedures.
  • Adhere to and maintain patient confidentiality and information governance at all times.
  • Ability to undertake Practice Nurse Duties including ECGs, Dressing, General Observations & Phlebotomy.

Meetings

  • It will be necessary to attend and contribute to various Practice meetings as requested. The only reason for not attending will be annual leave or sick leave.

Professional Responsibilities

Promote the welfare and safeguarding of children, young people and adults. Recognise professional and statutory responsibilities in the reporting of safeguarding, Deprivation of liberty and Prevent concerns.

Ensure appropriate measures are in place to protect self, other staff and patients in minimising and dealing with exposure to patient or environmental risk e.g. infectious diseases, harmful substances.

Raise unmet needs and service shortfalls with the Operations Manager, and provide constructive ways in which clinical services can be maintained and improved.

Participate in the practices annual appraisal scheme.

Participate in clinical supervision and training for staff in Respiratory care.

Maintain own continuous professional development and ensure mandatory training and personal development plan is kept up-to-date in line with the requirements of the Practice.

Maintain registration with the NMC and the revalidation process.

Maintain ARTP Registration

Practice within a legal and ethical framework that adheres to the NMC Code and local policies and procedures.

Attend and participate in staff, QOF and Clinical Governance meetings as required.

Protect personal and confidential information of patients, colleagues and visitors to the Practice and abide by the Practices Data Protection and Information Governance polices at all times.

Training and personal development

  • Training requirements will be monitored by yearly appraisal and will be in accordance with practice requirements. Personal development will be encouraged and supported by the Practice. It is the individuals responsibility to remain up to date with recent developments.
