Job summary
We have a permanent vacancy (2 days per week) for a Practice Nurse to deliver high-quality nursing services to our practice population. The role involves working as part of the practice multi-disciplinary team, providing care within an agreed scope of practice.
The Practice Nurse will take responsibility for a range of clinical areas including infection prevention and control, health promotion and chronic disease management. They will support the management team in the review and development of clinical policies and procedures.
Main duties of the job
To provide nursing services to the practice population including chronic disease treatment, screening and advice within the professional competence of the post holder. The post holder will always work to the Code of Conduct, as set out by the Nursing and Midwifery Council.

To take on responsibilities within the nursing team, these may include checking medical bags, Infection control measures, stock checks, health and safety tasks.

Assist medical colleagues in carrying out minor surgical procedures and the fitting of contraceptive devices
Provide telephone advice for patients and refer as appropriate to other members of the team
Take part in other appropriate patient care as requested (for example the need to chaperone medical colleagues during intimate examinations)
Phlebotomy is usually carried out by the HCA or Phlebotomists, but, depending on demand or sickness, there may be a need to see patients for venepuncture
To undertake chronic disease management and health promotion operated by the practice. This will depend on the skills of the post-holder
The areas may include:
General assessment and lifestyle advice, e.g. for patients with obesity or mental health problems
Assessment and ongoing checks for Hypertensive patients
Annual reviews for patients with cardiovascular disease
Annual reviews for patients with diabetes
Annual reviews for patients with asthma and COPD
Cervical cytology
Minor surgery procedures
Wound management
Leg ulcer dressing
All of the above to be recorded on the appropriate clinical templates within the clinical system.
