Job summary

To apply for the role, you will be an experienced Mental Health Nurse / Practitioner. The ability to independently prescribe would be desirable but not essential.

We are proud to have a diverse multidisciplinary team including a minor illness team, mental health nurses, paramedic, pharmacists, occupational therapists, social prescribers and physiotherapists.

Main duties of the job

The successful candidate will work closely with the team at the practice and be the first point of contact for patients with mental health problems, this includes but is not exhaustive to anxiety, depression, chronic enduring mental health diagnoses, stress related illness and dementia.

You will also be responsible for carrying out annual learning disability, dementia and mental health reviews.
