Job summary
We are expanding our nursing service and looking for a practice to work across our 2 practices at East Park Medical Centre and it's branch surgery in Halton.
We have a multidisciplinary team of clinicians who work together to support each other to deliver GP services to our practice population of over 10,000 patients in an area of Health Inequality. The practice is dedicated to helping to address these inequalities by adopting a Whole Health approach. We work hard with our community and local healthcare partners to engage patients in preventative health management.
We are using innovative technology to plan and deliver Long Term Condition Care.
We are looking for nurses who enjoy working with these types of communities and who have innovative ideas about how we can further improve access to care.
We also have a care home and community care bed service which is run out of East Park with a dedicated clinical team.
Main duties of the job
To provide nursing
services to the Practice population by assessment, planning, implementation and evaluation
of care
To ensure the provision of a high-quality Patient service within a safe, efficient
working environment by providing effective support to the Partners and staff and encouraging the development and
progress of East Park Medical Centre
Maintaining appropriate records
to ensure continuity and quality of care for Patients
To collaborate with other GP Partners/employees and Practice staff
on development and delivery
of PBC (Practice Based Commissioning) and enhanced service issues within East Park Medical Centre
To be involved in Practice development, identifying service
changes and developments in line with clinical
needs locally and work to develop these as part of PBC and other processes
Maintaining records of appointments and other administrative duties
To maintain skills
and undertake such training and development as may from time to time be required to maintain
personal competency
About us
Our clinical team is currently made up of a GP clinical
Lead, GPs, ANPs, ACPs, Mental health nurse, Pharmacists, Pharmacy technicians,
nurses and HCAs. Our non clinical team contains a management team, medical
secretaries, prescribing clerks and receptionists.
There are clinical meetings most weeks for all the clinical
team to cover clinical governance, safe guarding, palliative care and complex
presentations
We are an innovative practice embracing new ways of working
including new IT technologies.

There is a Director of Nursing who supports the Nursing leads at each site.
We encourage and support all members of our team in developing their careers through additional training and courses. A number of our Practice Nurses have attended course on blood test interpretation and are undertaking nurse prescriber courses.

Job responsibilities
· To provide assessment, treatment, screening, health education services and advice working in accordance with the NMC Code of conduct
· Provide nursing treatments to Patients independently or in participation with other clinical colleagues working to agreed protocols and guidelines
· Provide general and specific health screenings to the Practice Patients within agreed protocols and guidelines referring to other members of the clinical team as necessary
· To manage Patients with chronic diseases; running specific clinic sessions and maintaining robust call and recall systems
· Participate in national screening work such as cervical cytology and meet targets delivering the national immunisation and vaccination schedule
· Nursing procedures to include:
o Wound management
o Measuring blood pressure
o Routine injections as prescribed
o Cervical smears
o ECG’s
o Long term conditions-Asthma, Diabetes, COPD
o Vaccinations and immunisations
o Assisting with minor surgery and IUCD insertion
o Cold Chain
o Emergency equipment checks
· Provide advice to Patients for general health care and minor ailments, with referral to another clinician as necessary
· To participate in research within the Practice
· To be responsible for the efficient running of the treatment room and equipment
· To assist with the management of leg ulcers
· Select, carry out and interpret results of specific tests
· Ensure all statutory regulations (e.g. Infection control, Health & Safety, COSHH, etc.) are implemented and adhered to
· To maintain accurate, comprehensive records of all consultations and treatments in the Patient’s notes, both written and computerised
· Ensure accurate completion of all necessary documentation associated with Patient health care and registration with the Practice
· Ensure collection of statistical information is required for regular and ad hoc reports
· To have working knowledge of equipment used in the surgery
· Ensure compliance with Patient confidentiality at all times in line with current legislation
· Contribute to the development of new Patient services
· Inform your line manager of any reported or suspected failings in the provision of care within the Practice
