Job summary

Layton Medical Centre is a progressive and patient-focused GP practice dedicated to providing high-quality primary care to a diverse population. We are seeking an enthusiastic and experienced Practice Nurse to join our friendly, supportive, and multidisciplinary team. You will play a key role in delivering a wide range of nursing services within the practice, supporting patients with both acute and long-term conditions, and contributing to health promotion and disease prevention activities.

Main duties of the job

Delivering evidence-based nursing care, including chronic disease management for, diabetes, asthma, COPD, hypertension, Heart Failure

Carrying out vaccinations and immunisations, cervical cytology, wound care, and health checks.

Supporting the management of long-term conditions in line with QOF and best practice guidelines.

Providing health promotion advice and lifestyle support to patients.

Contributing to clinical audits, significant event reviews, and quality improvement initiatives.

Maintaining accurate and timely clinical records in accordance with practice policies and NMC standards.

Job description

Job responsibilities

Chronic Disease Management:

Managing patients with long-term conditions, including diabetes, asthma, COPD, hypertension, CHD, heart failure, and stroke/TIA follow-up. Using evidence-based care plans and protocols in line with QOF and NICE guidance. Performing annual reviews and medication monitoring.

Health Promotion & Disease Prevention:

Providing lifestyle advice on smoking cessation, weight management, diet, alcohol use, and physical activity. - Delivering NHS Health Checks and other screening initiatives.

Cytology & Womens Health:

Performing cervical smears in line with national screening guidelines. Advising on contraception and menopause if trained. Assisting with HRT monitoring.

Childhood Immunisations & Vaccinations:

Delivering the national childhood immunisation programme. Administering travel health advice and vaccinations. Providing seasonal and at-risk flu, COVID, shingles, RSV and pneumococcal vaccinations.

Long-Term Monitoring:

Venepuncture, ECG recording, blood pressure monitoring, and urinalysis. - Monitoring INR and other regular blood tests. - Supporting patients on DMARDs, anticoagulants, and other high-risk medications.

Professional Responsibilities:

Adhering to NMC Code of Conduct at all times. Maintaining accurate, timely, and confidential clinical records. Engaging in clinical audit, significant event analysis, and quality improvement activities. Keeping up to date with professional development and mandatory training. Supporting and mentoring junior staff and students where appropriate.

Teamwork:

Liaising effectively with GPs, ANPs, pharmacists, HCAs, and administrative staff. - Contributing to the smooth running and development of the practice.

Safeguarding:

All staff are required to act in such a way that at all times safeguards the health and well-being of children and vulnerable adults. You must be aware of and follow practice policies and procedures on safeguarding and take part in mandatory safeguarding training as required for your role
