We have a fantastic opportunity for an experienced General Practice Nurse to join our practice.

  • The Practice Nurse will act autonomously within their professional scope of practice providing care for patients face to face, telephone consultations and home visits.

  • You will be responsible for the provision of medical services to our patient population managing a caseload dealing with a wide range of health needs. You will be supporting & supported by the wide range of clinical and non-clinical team within the practice.

Main duties of the job

  • Manage a defined caseload of patients

  • Deliver chronic disease management (e.g. diabetes, COPD, asthma, hypertension)

  • Provide immunisations, cervical screening, wound care, and health promotion

  • Contribute to QOF achievement and clinical audits

  • Work collaboratively with the wider clinical and administrative team

    We are a very warm and welcoming practice of over 6,000 patients, providing quality healthcare for our patients in Oldham. We have 3 GP's, 1 ANP, 2 HCA's, currently 2 Practice Nurses.

    We are also a Minor surgery practice and also a training practice.

    Job description

    Job responsibilities

    • Practice independently in accordance with NMC code of conduct and with reference to practice guidelines
    • Maintain accurate documentation of care and utilisation of IT
    • Ensure continuation of own personal and professional development, facilitated by utilisation of clinical supervision networks.
    • Provide focus for nursing expertise within practice, advising nursing staff, appropriate to level of competence
    • Administer adult and child immunisations and vaccinations in accordance with national and local programmes. Is in competent in anaphylaxis and resuscitation techniques. Ensure safe storage, rotation and disposal of vaccines and durgs and adequate stock levels are maintained.
    • Takes a lead in areas such as Asthma and COPD management, undertakes the establishment and running of disease management clinics and provides specialist nursing care for the patients or communities that practice serves.
    • Management of chronic diseases which includes asthma, hypertension, COPD, CVA/TIA, CHD, and CHD risk assessment and management. Initiates the process of diagnosis with patients suspected to have chronic disease

    • Initiates and carries our programmes of Health promotion and screening, cervical and sexual health screening.
