Job summary
We are looking for an ANP/ACP to join our nursing team. The role is part-time with hours to be negotiated.
To ensure the delivery of an effective triage service by acting as an autonomous practitioner working independently and in conjunction with other health care professionals to assess, diagnose, and treat the conditions of patients within primary care initiating direct referrals as appropriate.
Provide professional advice that is evidence and research based, to patients, carers and colleagues and to ensure the maintenance of clinical excellence.
Follow national and local clinical guidelines and governance principles.
To provide education and training to other staff and students as needed.
Main duties of the job
Working as an independent practitioner preferably within a primary care setting or similar. Management of patients with long term conditions.
Minimum 1 year + experience at a senior level.
At least 2 years primary and community nursing experience (or similar).
Nurse-led management of minor illness, minor ailments and injuries.
Nurse-led triage.
Compiling protocols and clinical guidelines.
Accountability of own role and other roles in a nurse-led service.
Clinical governance issues in primary care.
Wider health economy
Advanced Clinical Assessment Skills.

We are a 5 partner dispensing Practice located in
the heart of the market town of Pershore with around 10,600 patients in Pershore and the surrounding villages. Our aim: to provide
high quality modern medical care; to enable easy access to a full range of

We have 9 GPs, 1 GP Registrar, 2 Advanced Nurse Practitioners, 5 Specialist Practice
Nurses, 2 Practice Nurses, 1 Clinical Pharmacist, 1 Pharmacy Technician, 1 Healthcare Assistant and 1 Nurse Associate.
Staff- a great team of helpful professionals including ANPs who triage same day urgent appointments, practice nurses whose duties include the management of chronic disease and clinical pharmacists who deal with all medication queries, hospital discharge letters and assist with GP workload.
Building - we have a modern, spacious building with staff parking. We have excellent clinical facilities including a minor surgery suite. We also have a large bright and comfy staff room where staff meet daily for networking and coffee.

We are part of the Pershore and Upton PCN and work closely with our neighbouring practices.

- Prescribe medicines and appliances as determined by patient need.
- Directly admit patients to secondary care hospital in acute medical need.
Refer patients to other medical specialities or to other members of the multidisciplinary team for assessment if deemed appropriate.
- Interpret diagnostic tests and routine clinical procedures.
- Advise and support others as needed.
