We are looking for an experienced Advanced Nurse/Care Practitioner educated to masters level in Advanced Health Assessment, holding a Prescribing qualification.

Working as part of our urgent care team, you will work autonomously yet collaboratively to provide advice, diagnosis and treatment both on the phone and face to face in surgery or within the community. Part of the role includes the review of patients with a Nursing Home setting, therefore you will need the use of your own car.

Main duties of the job

Provide the choice of direct access to a ANP/ACP, both in the practice and over the telephone, and dependent on the individual, at home and in the community for the general practice population with an acute or chronic health problem

Provide a first point of contact within the practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan

To prescribe safe, effective and appropriate medication as defined by current legislative framework. Registered Independent Prescribing.

Offer a holistic service to patients and their families, developing where appropriate an on-going plan of care/support, with an emphasis on prevention and self-care.

Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills.

Work with multi-disciplinary team within the Practice and with other service providers to promote integrated and seamless pathways of care

Assess acute and chronic illness in the surgery and in Nursing and Care homes.
