Job summary
At Hereford Medical
Group we are currently looking to hear from any qualified, enthusiastic,
motivated and experienced Pharmacists to join our existing pharmacy team.
This is a 30-37.5 Hours per week fixed-term contract to cover maternity leave
for 12 months.
We do not require the candidate to
have experience of working within a general practice as within this role you
will be supported by our Senior Practice Pharmacist and Pharmacy Team. You will
be working closely with our GPs and the pharmacy team in the management
of medicines on transfer of care and systems for safer prescribing, and will
also have a role in managing patients long-term conditions by undertaking
structured medication reviews (SMRs).
Main duties of the job
The successful
candidate will be working as part of our multi-disciplinary team providing
expertise in clinical medicine management, completing medication reviews with
patients and producing recommendations for the GPs on prescribing and monitoring.
They will be
providing a telephone help line for patients with questions, queries and
concerns about their medicines and hold face-to-face clinics for our patients
that are requiring structured medication reviews.
The candidate will
also be confident, have excellent organisational skills, be able to self-manage
and be IT proficient.
We have a large team
that supports one another supported by our dedicated Safeguarding Officer
supporting with referrals and the management of our vulnerable patients.
