Job summary
We are looking to recruit to the post of a Care Coordinator to work within our Primary Care multidisciplinary team. The successful candidate will play a key role in proactively identifying and working with people including the elderly/frail, vulnerable and those with log term conditions, to provide co-ordination and navigation of care and support across health and care services.
They will work closely with GPs and Practice Teams, making sure that support is made available to people, enabling them to understand and manage their condition. They will enable people to access services to meet their health and wellbeing needs.
You will work alongside other professionals to provide a personalized approach to care and support needs
You will work with a diverse population , and have the ability to work effectively in avaried and sometime challenging environment. You will have excellent communication and interpersonal skills, and show empathy.

- Care Coordinators will:
- Work with people, their families and carers, to improve their understanding of their condition.Identify carers and help them access services to support them
- Support people to develop and review personalised care plans to manage their needs and achieve better healthcare outcomes
- Ensure that people have good quality written or verbal information to help them make choices about their care
- Provide coordination and navigation for people and their carers across health and care services
- Develop an in-depth knowledge of the local health and care infrastructure and know how and when to enable people to access support and services that are right for them
- Work collaboratively with GPs and other primary care professionals within the PCN to proactively identify and manage a caseload and where appropriate refer back to other healthcare professionals within the PCN
- Refer onwards to social prescribing link workers and health and wellbeing coaches where required and to clinical colleagues where there is an unaddressed clinical need
- Support the coordination and administration of PCN MDT meetings, by
- Arranging meetings, sending invitations , co-ordinate, take minutes
- Raise awareness of how to identify patients who may benefit from shared decision making and support
