Job summary

North Wilts Border Primary Care Network is a well-established group of practices and are proud to offer the highest standard of patient-centred healthcare covering Malmesbury, Sherston, Royal Wootton Bassett, Purton and Cricklade.

We are looking to recruit a Paramedic to join our existing Paramedic team, to work across the GP practices.

Main duties of the job

The ideal candidate will have experience in managing patients with acute and chronic illnesses, experience with history taking, clinical assessment, diagnosis, treatment, face to face and telephone/video consultations, home visits and minor illness care. The majority of your time will be visiting patients in their homes, with some time seeing patients at the practices.

You will be working closely with GPs at the practices. We are friendly, approachable, and supportive of our staff and encourage our staff to reach their full potential.

About us

This is a full-time role and HCPC registration and experience in a Primary Care setting is desirable but not essential, training will be provided but safe clinical decision making, and patient care are paramount.

Essential to have excellent computer, patient /people skills along with an eye for detail and be prepared to be flexible being a key member of the clinical team influencing and developing the role.

AIMS OF POST

The Specialist Practitioner will utilise skills such as those of paramedics to support the first contact needs of patients in unscheduled care. Supported by GPs and triage teams from the practices, the Specialist Practitioner will triage home visits to either facilitate an early presentation in A&E, thereby reducing the likelihood of admission, or referral to other services as necessary. The Specialist Practitioner will independently provide definitive care that may not require further intervention of a GP.

JOB SUMMARY

The post holder will liaise with individual surgeries to manage home visit requests on a daily basis and triage such visits with a view to an improved and more rapid response and a reduction in unplanned admissions. The role will involve travel in the locality and between surgeries and remote access to IT systems.

The post holder will be responsible for planning, implementing, evaluating and prioritising their workload.

MAIN RESPONSIBILITIES

Completing emergency assessments of elderly patients in their home and at the surgery when presenting for urgent care, including those patients with long term conditions.

Undertaking an in-depth patient examination and assessment beyond existing JRCALC (current practice framework for paramedics) guidelines.

Treating minor illness and minor injury in the patients home and at the surgery.

Carrying out and interpreting diagnostic tests within the limits of personal clinical competence.

Administering medications within the remit of local Prescribing Guidelines.

Taking a holistic approach to patient care, bringing together their medical, social and psychological needs.

Encouraging the promotion of health education in the community.

Empowering patients to take responsibility for managing their own care and treatment where it is safe and appropriate to do so.

Referring patients to alternative health and/or social services if necessary.

Communicating regularly and effectively with GPs and the surgery teams

Communicating and collaborating with members of the Community Integrated Team as needed.

Writing up full and discursive notes of patient encounters in the practices clinical system.

Breaking down existing professional boundaries and encourage collaboration.

Identifying and providing alternative care pathways for the provision of emergency care.

Undertaking audit and research; be personally responsible for self-directed learning.

  • a)More than a minimum of 5 years post-registration experience. This assumes that the paramedic has been through a 2 year mentorship to a Band 6 role (Agenda for Change) then has 3 years further clinical experience.
  • b)Is able to demonstrate Level 7 knowledge and skills. By this they mean that the candidate should have completed academic attainment at postgraduate level (level 7), within 6 months of taking op the post if not already achieved.

Desirable
