Working across the Primary Care Network comprising
of North Swindon Practice (employing practice) and Ashington House Surgery.
Starting salary of £24.00 per hour plus NHS
Pension
Full-time permanent post, 37.5 hours per week
5 weeks annual leave plus 1 week study leave (pro
rata) plus Bank Holidays
Training practice offering mentorship and support for
professional development, which could include becoming an independent
prescriber or gaining a postgraduate qualification in clinical pharmacy.
Main duties of the job
The successful candidate will work closely with
our Senior Clinical Pharmacist and will potentially have a patient-facing role, working
directly with patients to promote safe and effective use of medication and
self-care. The role will involve working
as part of a multi-disciplinary team to develop and run processes for medicines
management on transfer of care and systems for safer prescribing

In association with our Partner Practice in Brunel 3 PCN, we are looking for an additional Pharmacist to join our team of Pharmacists/Technicians who can help us develop medicines optimisation services within our Primary Care Network.

- KEY ASPECTS OF THE ROLE:
- Work as part of a multi-disciplinary primary healthcare team in both a back office and patient-facing role.
- Provide pro-active support for effective medicines management for patients suffering from chronic diseases.
- Within the limits of professional competency manage medication for patients with common minor ailments.
- Undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
- Provide primary support to practice staff with regard to prescription and medication queries.
- Support the practices repeat prescribing systems and deal with acute prescription requests and medicines reconciliation on transfer of care.
- Implement systems for safer prescribing, providing expertise in clinical medicines advice.
- Provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.
