Job summary

We are seeking a motivated Clinical Pharmacist to join our NHS GP practice team. The successful candidate will play a vital role in supporting safe, effective, and patient-centred prescribing, working closely with GPs and the wider multidisciplinary team.

You will be responsible for delivering structured medication reviews, managing long-term conditions, and providing expert medicines advice to both patients and clinicians. The role also includes supporting improvements in prescribing processes and ensuring compliance with guidance from NHS England.

Main duties of the job

  • Undertake structured medication reviews and optimise prescribing for patients

  • Support the management of long-term conditions through effective medicines use

  • Provide expert medicines advice to GPs, nurses, and the wider practice team

  • Identify and reduce medication-related risks, including high-risk drug monitoring

  • Process prescription requests and support medicines reconciliation following discharge

  • Work in line with guidance from NHS England and local prescribing policies

  • Support quality improvement initiatives and prescribing audits

    Job description

    Job responsibilities

    • Work within the Pharmacy Team and with other clinical pharmacists to develop and ensure a safe efficient medicines optimisation programme including repeat prescribing, medication review and medicines reconciliation services.

      Carry out face-to-face clinics providing management and review of patients with asthma, hypertension, diabetes, and other clinical areas within the pharmacists scope of competence.

      Proactively support GP practices to achieve performance targets set by the ICB, PCN, and national frameworks, including QOF indicators, through clinical optimisation, audits, and patient-focused interventions.

      Coordinate and document practice responses to MHRA safety alerts, including patient identification, risk assessment, clinical review, and communication with relevant stakeholders.

      Help achieve efficiencies and effectiveness within the prescribing activities of the PCN group of GP practices.

      Promote high quality cost-effective and evidence-based use of medicines within the PCN.

      Work within the Pharmacy Team and with other clinical pharmacists to respond to medication-related queries generated by patients, GPs, other health care professionals and staff.

      Become proficient with TPP/ SystmOne GP prescribing system and supporting IT to record personally generated information and maintain a database of information relating to the work done in the practices.

      Undertake clinical medication reviews either face to face or via telephone support on identified stratified-risk patients that may include residents in care homes, in their own, with long-term conditions, and/or learning disabilities.

      Identify and produce recommendations for medication optimisation for senior clinical pharmacists, GPs and other multi-disciplinary team members.

      Work with care home staff to improve safety of medicines ordering and administration if required.

      Work to encourage good management of common/ minor/ self-limiting ailments, including promotion of self-care and homely remedies.

      Contribute to the safety of drug monitoring processes and drug alerts.

      Analyse, interpret and present medicines data to highlight issues and risks to support decision-making.

      Advocate and implement local, regional and national formulary recommendations, including public health campaigns.

      Liaise with all members of the local PCN including community pharmacies and community nursing team, secondary care providers and regional CCG teams on prescribing-related matters.

      Aim for good collaborative working practices and develop relationships with other professionals across PCNs and the wider health and social care system.

      Work with your line manager to undertake continual personal and professional development, taking an active part in reviewing and developing the role and responsibilities.

      Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety.

    • Knowledge of repeat prescribing processes and medicines reconciliation
    • Experience of working collaboratively within a multidisciplinary healthcare team

    • Good understanding of NHS primary care systems and processes, including guidance from NHS England

    Desirable

    • Previous experience working in a GP practice or Primary Care Network (PCN)
    • Experience managing long-term conditions (e.g. asthma, diabetes, hypertension)

    • Experience as an Independent Prescriber or working towards this qualification
