Job summary

An opportunity has arisen to join the nursing team at Woodlands Medical Centre as a Healthcare Assistant.

Woodlands Medical Centre is based on the outskirts of Didcot and has a population of 18,850 patients which continues to grow with the ongoing housing development in the area.

Main duties of the job

The role of the Healthcare Assistant is essential in supporting our patients with undertaking investigations such as blood tests, ECGs, Annual Health Reviews for long term conditions, flu immunisation and other injections. These investigations inform the baseline management of our patient population.

There are opportunities to develop your role and work to support our specialist clinics in Dermatology, coil insertion and minor surgery as well as to train up in wound management and dressing clinics.

Job responsibilities

JOB PURPOSE

To work as part of the Primary Health Care Team providing high quality nursing care to meet the holistic needs of the Practice population. To work to agreed Practice standards and protocols for the delivery of patient care.

PRIMARY DUTIES AND AREAS OF RESPONSIBILITIES

KEY ROLE CLINICAL NURSING PRACTICE

Undertake procedures to support patients including:

Venepuncture

Blood Pressure checks, height, weight, BMI, urinalysis,

ECGs

NHS health checks

Chronic disease health checks

Assisting in minor procedures/minor surgery

Chaperoning other clinical staff

Supporting patients who are anxious or distressed

Immunisations eg seasonal influenza.

Injections eg B12

Support patients in their physical, mental and social wellbeing by:

Engaging in opportunistic or formal Health Promotion where appropriate.

Working with the Practice to support vulnerable patients.

Communicating or escalating concerns to a senior member of the team in a timely manner

Encouraging and supporting patients to address issues which affect their health and social well-being.

Works under indirect supervision of the Senior Practice Nurse.

Is accountable to the Senior Practice Nurse and the Partnership/Leadership Team for their actions in undertaking delegated work.

Provides support for other members of the primary health care team and contributes towards the co-ordination of team activities and the development of the Practice as a whole.

Refers to more senior members of the Primary Health Care team as appropriate.

Works within all relevant practice policies and procedural guidelines for example: infection control, chaperoning.

Is required to keep competencies up to date and partake in ongoing training and appraisal process for continuing professional development.

Promotes effective communication and relationships within the team.

Works collaboratively as a team member to achieve the aspirations and objectives of the Practice as a whole.

Actively participates in clinical and practice meetings.

Contributes to the development, monitoring and evaluation of care delivery.
