Job summary

An exciting opportunity has arisen for an experienced Nurse to join our large (20,500 patients), friendly, rural, dispensing practice. This is a two year training programme. The successful applicant will spend one day a week at UCLAN undertaking the MSc in Advanced Clinical Practice and must already hold a nursing BSc and be able to work a minimum of 30 hours per week, although full time hours will be considered. We want friendly and personable candidates who have the communication skills, enthusiasm and aptitude to work alongside our existing experienced team of Advanced Nurse Practitioners, running minor illness clinics and working alongside our GPs and Clinical Pharmacists as part of our Acute Team. We pride ourselves on our collaborative and supportive working environment in which all staff are valued and ongoing training supports staff in their career development. Applicants need to be team players who can cope under pressure, and who will be able to work independently and flexibly within our dynamic and friendly team. Experience in chronic disease management and community work would be beneficial, but not essential, due to the home visiting aspect of this role.

Main duties of the job

- 8-10 sessions per week

- Telephone, video and face to face consultations

- Telephone triage

- Home visits including to nursing and care homes

- Working with extremely supportive GPs, ANPs, Practice Nurses and Practice Pharmacists

- Referral to community and secondary care services

- Managing a variety of acute and chronic medical conditions

- Assisting with training of clinical and non-clinical colleagues.

About us

We are a large, friendly, extremely supportive training practice with an extensive multi-disciplinary team which includes; GPs, Trainee GPs, ANPs, First Contact Practitioner, Paramedic, Clinical Pharmacists, Chronic Disease Nurses, Treatment Room Nurses, HCAs, Phlebotomists Mental Health Practitioners, Health and Well Being Worker and Social Prescriber. Daily clinical meetings ensure excellent communication and support and provide a daily opportunity for MDT discussion and learning. We have previously successfully trained and supported 4 ANPs, all of whom have gone on to permanently join our team. We provide dedicated mentoring from GP and ANP colleagues and generous protected study time.

Clinical practice:

To deliver a high standard of patient care as an advanced clinical practitioner in general practice, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base.

Use expertise and decision-making skills to inform clinical reasoning approaches when patients present with differentiated and undifferentiated presentations. Making use of skills in history taking, holistic assessment, identifying risk factors, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan. Working in partnership with individuals, families and carers.

Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports.

Initiate, evaluate and modify a range of interventions which may include lifestyle advice and care through the use of health promotion, health education and screening.

Where the post holder is an independent prescriber to prescribe safe, effective and appropriate medication as defined by current legislative framework.

To prioritise health problems and intervene appropriately, including initiation of effective emergency care.

Recognise and manage anaphylaxis within current UK Resuscitation Council guidelines.

Perform practical basic life support procedures and techniques within current UK Resuscitation Council guidelines.

Work collaboratively with multi-agency and inter-professional resources, developing, maintaining and evaluating links to manage risk and issues across organisations and settings.

Act as a clinical role model for developing and delivering care that is responsive to changing requirements, informed by an understanding of local population health needs, agencies and networks.

To clearly communicate the health status of the patient using appropriate technology and format.

To work within the practice guidelines, policies and protocols.

Leadership and Management:

Proactively initiate and develop effective working relationships, fostering clarity of roles within teams, to encourage productive working.

Evaluate own practice, and participate in multi-disciplinary service and team evaluation, demonstrating the impact of advanced clinical practice on service outcomes.

Actively engage in peer review to inform own and others practice, formulating and implementing strategies to act on learning and make improvements.

Actively seek feedback and involvement from individuals, families, carers, communities and colleagues in the co-production of service improvements.

Demonstrate team leadership and resilience when managing situations that are unfamiliar or unpredictable.

Continually develop practice in response to changing population, health need and promote the sharing and adoption of best practice.

Participate in the education and training of students of all disciplines.
