We
are seeking a dynamic and highly motivated ACP or ANP to join our team,
working either full or part time. The postholder will undertake a varied range
of clinical duties, supported by our experienced clinical and administrative

Working
autonomously within your scope of practice, you will assess, diagnose, treat,
and manage patients with a range of acute and long term conditions, playing a
pivotal role in delivering high quality primary care.
Main duties of the job
The ACP or ANP will play a key role in delivering high quality
clinical care within the practice. The role involves the assessment, diagnosis,
treatment, and or referral of patients presenting with undifferentiated and
undiagnosed conditions. The postholder will work independently within
their professional scope of practice, with regular debriefs and access to a
supervising GP for advice throughout the day.
Main Responsibilities
- Deliver a high standard of patient care using
advanced autonomous clinical skills supported by a broad and indepth
theoretical knowledge base.
- Assess, diagnose, and manage patient
conditions, initiating appropriate referrals when required.
- Manage acute illnesses, long term conditions,
and minor ailments confidently and safely.
- As an independent prescriber, prescribe safe,
effective, and appropriate medication in line with legislation and professional
guidelines.
- Contribute to the practices triage and urgent
care systems.
- Work collaboratively with GPs, nurses, and
wider healthcare professionals to ensure seamless, highquality patient care.
- Promote health and wellbeing using health
education, screening, and therapeutic communication skills.
- Maintain uptodate clinical knowledge and
skills, with full support from the practice for ongoing training and
professional development.

are a well established, friendly, and supportive GP surgery serving
approximately 13,500 patients across a large rural catchment area and
are proud to be part of the North Shropshire Primary Care Network. Our
multi disciplinary team includes GPs, Advanced Nurse Practitioners, Practice
Nurses, a Nurse Associate, Healthcare Assistants, Clinical Pharmacists, and
allied health professionals such as a Cancer Care Coordinator, First Contact
Physiotherapist, and Paramedic. Together, we deliver high quality, patient centred
