Reception Health Care Navigator / Secretary

Job summary

An exciting opportunity has arisen for a motivated, enthusiastic, and hardworking individual to join Wolseley Medical Centre in a dual role as a Health Care Navigator and Secretary, working across both the Reception Team and the Administrative Team.

This is a hybrid position, providing vital support to both the reception team and the administrative team. The successful candidate will be expected to balance patient-facing duties with a range of administrative and secretarial responsibilities.

Main duties of the job

Reception / Health Care Navigation:

  • Act as the first point of contact for patients.

  • Provide care navigation by signposting patients to the most appropriate service or clinician.

  • Deliver high-quality customer service both face-to-face and over the phone.

  • Manage appointment bookings and patient enquiries.

Secretarial / Administrative:

  • Provide comprehensive secretarial and administrative support to the multidisciplinary team.

  • Process electronic and paper-based information accurately and in a timely manner.

  • Liaise with internal team members and external agencies, including hospitals and community services.

  • Use the electronic referral service (ERS) and assist with tasks related to referrals, correspondence, and other administrative processes.

Job description

Job responsibilities

The following are the core responsibilities of the Care Navigator and Medical Secretary. There may be, on occasion, a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels.

  • Process and effectively signpost patients to the appropriate healthcare professional, depending on the presenting condition
  • Answer incoming phone calls, transferring calls or dealing with the callers requests appropriately
  • Process patient requests for appointments
  • Process repeat prescription requests
  • Initiate contact with and respond to requests from patients, team members and external agencies
  • Photocopy documentation as required
  • Input data into the patients healthcare records as necessary
  • Manage all queries as necessary in an efficient manner
  • Support all clinical staff with general tasks as requested
  • Scan patient-related documentation and attach scanned documents to patients healthcare records
  • Complete opening and closing procedures in accordance with the duty Rota
  • Undertake all mandatory training and induction programmes
  • Typing letters, reports and associated documentation as required
  • Liaising with external agencies such as hospitals and community services, ensuring referrals are processed efficiently
  • Maintaining an accurate referrals database
  • Actioning all incoming email
  • Scanning patient related documentation and attaching scanned documents to patients healthcare records
  • Processing referrals using the electronic referral system (ERS)
