Job summary

The Cranborne practice is seeking a dedicated and compassionate Advanced Nurse Practitioner (ANP) to join our healthcare team. The ANP will play a crucial role in delivering high-quality first pont of contact patient care, utilising advanced clinical skills and knowledge to assess, diagnose, and manage patient health issues. This position requires a strong foundation in anatomy, patient care, and medication administration to ensure optimal health outcomes for our patients.

Main duties of the job

  • Conduct comprehensive assessments of patients, including physical examinations and medical histories.
  • Develop and implement individualised care plans based on patient needs and clinical guidelines.
  • Administer medications and monitor patient responses to treatments.
  • Collaborate with other healthcare professionals to provide holistic care and support for patients.
  • Educate patients and their families about health conditions, treatment options, and preventive measures.
  • Maintain accurate and up-to-date patient records in compliance with regulatory standards.
  • Stay current with advancements in medical practices and participate in continuous professional development.

About us

The Cranborne practice is a forward thinking and motivated practice over two sites, one in the little village of Cranborne and the other at Lake road surgery Verwood. Based on the edge of the New forest in East Dorset.

We have approx. 13,500 patients over our two sites. The Cranborne practice is part of Crane Valley PCN.

Job responsibilities

Clinical Nursing Practice

  • Work as part of a multi-disciplinary team to deliver safe and high quality care.
  • Carries out clinical nursing practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care.

Delivery of care in line with NSF, NICE guidelines and evidence-based care

  • Triage and treat patients, performing specialist assessment of their needs and make referrals as necessary to other members of the primary and secondary health care teams as appropriate.

Co-ordinate the planning and delivery of care ensuring patients and carers/relatives are involved.

  • Prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.
  • Triage and treat patients wishing to see a health care professional, making any necessary referrals to other members of the primary health care team
  • Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations.
  • Advise patients on general health care and minor ailments, with referral to other members of the primary and secondary health care team as necessary
