Job summary
The Practice Nurse will be responsible for a
number of clinical areas within the Practice, these include, infection
prevention and control, health promotion, chronic disease management, well
women and well man clinics, supervising and assessing students and junior staff
members in their CPD as well as actively supporting the practice management
team in the reviewing and delivery of clinical policy and procedures and QOF.
To be responsible for the delivery of
practice nursing services, working as part of the practice multidisciplinary
team, delivering care within their scope of practice to the entitled patient
population.
Victoria
Medical Centre (VMC) must ensure that the post holder has access to appropriate
clinical supervision and an appropriate named individual in the organisation to
provide general advice and support on a day-to-day basis.
Main duties of the job
The
following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to
carry out other tasks. This will be dependent upon factors such as workload
and staffing levels: Key
Responsibilities:
- Identify, manage, and
support patients at risk of developing long-term conditions, preventing
adverse effects on the patients health.
- Provide chronic disease
clinics and multi-morbidity clinics, delivering patient care as needed
and referring patients to secondary/specialist care as required
- Administer childhood
immunisations in line with current PGDs.
- Undertake a programme of
cervical cytology screening.
- Provide travel
vaccinations and administrative services.
- Provide hosiery
measurements as required.
- To administer medication
for patients in accordance with PGDs/PSDs and within own scope of
practice.
- Undertake opportunistic
screening including BP monitoring, urinalysis, weight monitoring,
phlebotomy, ECGs, Doppler, and spirometry.
- Offer suture removal, wound
care, and management of patients with leg ulcers.
- Undertake ear irrigation
and provide ear care advice.
