Clinical Pharmacist
Permanent position, Part time 25 hours per week

Main duties of the job
Main Duties and Responsibilities
Manage and promote the repeat dispensing service
2.Increase safety mechanisms such as audit and PDSA
cycles
3.Implement drug withdrawals and alerts
4. Reduce inappropriate poly-pharmacy and wasteful
prescribing through clinical medication reviews
5. Contribute to reductions in medication related hospital
admissions and readmission bysupporting patients to get the best outcome
from their medicines and identifying and addressing medicine related issues
6. Reconcile medicines following hospital discharge and work
with patients and community pharmacists to ensure patients receive the
medicines
7. Work within the Group to undertake medication reviews
particularly in high risk groups such as frail elderly, substance misuse and
patients on high risk medicines
8. Improve prescribing practice through educational support
for all prescribers within thepractice
9.Run and oversee the YMG Anticoagulation service
using the surgerys protocol
10. Lead on where changes in evidence require changes in
prescribing across patientpopulation eg where a drug is withdrawn or
indications change
