Job responsibilities
MAIN DUTIES AND RESPONSIBILITIES:
To provide Health Care Assistant services to the practice population.
Supported by the nursing teams, deliver care within the boundaries of role and in accordance with specific practice guidelines and protocols
Responsible for the care delivered, demonstrating critical thinking and skills in clinical decision making within the parameters of their qualifications and role requirement
Focus up on supporting patients to be healthy, by assessing their needs, providing HCA care, health education advice, screening activities, implementing care at the surgery and in the community settings when required, running a variety of clinics
Maintain clinically accurate records.
Work collaboratively with the general practice team to meet the needs of patients and support the delivery of policy, protocols and procedures, providing HCA guidance as required.
To make professional decisions, providing safe, evidence -based, cost effective individualised patient care to include; health checks, manual and automatic blood pressure, manual and rhythm pulse, temperature, height, weight, BMI and blood sugar levels.
To be involved in taking and preparing pathology samples which will include, venepuncture, INR and urinalysis.
Undertake ECG's
Assess, plan, develop, implement and evaluate programmes to promote health and well-being and prevent adverse effects on health and well-being.
Support patients to adopt health promotion strategies and encourage principles of self-care.
Offer holistic service to patients and their families, developing where appropriate an on-going plan or care/support, with an emphasis on prevention and self-care
Weight Management Advice.
signposting to relevant healthcare providers to assist with the promotion of healthy living.
Ear Care; providing education and advice in relation to basic ear care. Undertake ear syringing when clinically indicated.
Administer B12, Influenza, Pneumonia and shingles injections under the provision of Patient Specific Directives.
Chronic Disease Management; undertake appropriate investigations and clinical observations following practice guidelines and protocols for patients with chronic diseases CHD, diabetes and Hypertension.
Wound Management; meet the needs of the patients presenting for wound care and use dressing formulary and care plan initiated and reviewed by the practice nurse. Provide basic wound care and removal of stiches and clips.
Emergency Care; prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
Adult and child safeguarding; recognise patients who may be at risk and raise concerns appropriately.
Infection Control; ensure that policy and procedures are always adhered to.
Stock Control; ensure that adequate stock levels in all clinical rooms are maintained and rotated. Ensure daily, weekly and monthly checks are completed.
Participate in audits where appropriate.
The successful candidate may be required to work across the sites on occasions so a degree of flexibility is required, plus some extended hours and Saturdays will be expected on a rota basis at some point.
