Job summary

You will work collaboratively within a Multi-Disciplinary Team or independently to meet the needs of patients, ensuring the highest standards of care and supporting the delivery of policy and procedures.

You must work at all times within your own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC) and use your own judgement, knowledge and common sense in your day-to-day activities.

It is expected that you will fully utilise all opportunities for learning under the guidance of PCN leadership team and will demonstrate evidence of professional development and increasing responsibility for patient care and related duties

Main duties of the job

You will be responsible for the provision of skilled nursing care to the practice population working as part of Kettering and South West Rural Primary Care Network (PCN). You will work as an active member of the multidisciplinary team supported by practice nurses, the PCN Manager and Clinical Director. You will deliver care within the boundaries of your role, focusing on supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities.

o New patient checks

o Well man/woman checks

o Advice for healthy eating, exercise, alcohol consumption and smoking

Perform routine nursing tasks such as:

o Venepuncture

o Recording ECG

o Spirometry

o Chaperoning and assisting patients

o Dressings and wound care

Provide a holistic approach to wound management and implement wound care in line with current evidence-based guidelines

Offer advice about childhood and adult vaccinations and administer vaccines according to current guidelines and legislation

Advise and administer vaccinations for patients travelling abroad

Assist in the provision of treatment plans to meet family planning needs

Administer and monitor medication consistent with protocols, standards, and legislation

Assist in the provision of nursing care within the context of primary care for patients with mental health problems

Delegate aspects of care as appropriate

Monitor and manage maintenance of stock and equipment, reporting to nurses / PCN Manager as required

Perform other clinical tasks as trained and agreed to meet the needs of the Practice. This may include:

o Chronic disease management reviews including asthma, diabetic, non-diabetic hyperglycaemia cardiovascular disease, high risk of cardiovascular disease and hypertension

o Annual reviews including learning disability and mental health reviews

o Consultations for diabetics including lifestyle advice, BP, weight, height, foot assessments and referring to suitable registered practitioners

o Travel risk assessments using NaTHNaC website and other relevant websites, ability to document vaccinations and advice required, seeking advice from suitable registered practitioner

Administer IM vaccinations including, NHS selective programme, NHS additional vaccinations and NHS supported travel vaccinations which may include: Covid, Shingles, Pneumococcal, Influenza, Hepatitis A, Hepatitis B, Typhoid, Boostrix, Meningitis ACWY, HiB, Revaxis under a Patient Specific Directive

Perform oral contraception checks using clinical IT template referring to GP or family planning nurse as appropriate

Administer injectable contraception including under a Patient Specific Directive following the late injection guidelines if appropriate, and refer to GP or suitable qualified practitioner if required

Perform more complex wound care following a documented plan of care completed by a suitable qualified practitioner

Prepare specimens for investigation by the pathology laboratory

Assist in the provision of minor surgery and surgical procedures assist in raising awareness and promotion of health within own competence

Give accurate and appropriate information to patients within own competence

Assist the patient and other clinicians in the delivery of care in urgent or emergency situations, including initiation of effective emergency care

Act as chaperone and assist patients

Prepare and maintain environments and equipment before, during and after patient care interventions according to Infection Control guidelines

Undertake infection control monitoring

Assist nurses with the maintenance of stocks of vaccines and other drugs in according to current guidelines and practice policy

Maintain and control stock levels of clinical equipment and supplies, reporting to the senior nurse as required

Physically assist in the transfer and movement of materials and equipment

Maintain accurate and complete records and other documentation in accordance with Practice protocol

Be aware of and recognised issues relating to safeguarding for both adult and child and reports any concerns to the appropriate person
