Primary Care Pharmacist
Stourside Medical
Practice is seeking an experienced and dynamic Primary Care
Pharmacist to join our busy yet friendly team, serving a patient
population of approximately6,000.This is a new zero hours
position with an initial expectation of one day a week. Once fully up to speed
with our way of working and the role it may be possible for this day to be
remote working.

a proactive and motivated pharmacist to work collaboratively with our clinical
team. The successful candidate will help us deliver an effective, efficient,
and high-quality medicines management service for both our patients and our
colleagues.

Stourside Medical
Practice is a purpose built, modern surgery based in Halesowen town centre with
a good road network easily accessible from Birmingham and surrounding areas. We
are a GMS training practice and operate from an Emis Web/Docman software
platform. We are a dynamic and digitally minded practice continually looking to
evolve and can offer flexible sessions to suit your lifestyle.
Main duties of the job
The successful candidate will be a Prescribing
Pharmacist with experience of working in primary care. They will play a central role in safe, effective, evidence-based
medication management for our patients. They will support high-quality
prescribing across the practice through prescription signing, structured medication
reviews, Blood Pressure Management, medicines reconciliation through Docman, and safety searches and audits to optimise prescribing
and improve patient care.

Provide safe, effective
prescribing aligned with clinical guidelines and the local formulary. Undertake
structured medication reviews, medicines reconciliation, and monitoring to
support long-term condition management and medicines optimisation. Complete
prescription signing accurately and efficiently. Carry out regular safety
searches and audits to maintain high standards.

We have an experienced team of salaried GPs and practice nurses with the practice actively involved in a thriving Primary Care Network with a variety of allied health professionals supporting the practice including Pharmacists, Social Prescribers and First Contact Physiotherapist We are an approved Training Practice with 2 trainees in post and an Advanced Diabetes Practice with an experienced diabetes specialist Nurse employed directly by the practice.
