We have an amazing opportunity for a highly motivated and dedicated Paramedic to join our established multidisciplinary team of clinicians, supporting our innovative care home, acute and home visiting service, whilst we in turn support your personal development with annual appraisal, training and education.
Work as an effective and responsible team member supporting others and exploring the mechanisms to develop new ways of working to benefit the practices and the patients.
Working within the Enhanced Health in Care Homes (EHCH) model we are supporting people living in care homes who can expect to receive the same level of support as if they were living in their own home.
Supporting our housebound patients to improve the management of LTCs and help reduce admissions.
Triaging and attending acute home visits in rotation with the clinical team.

Main duties of the job
Job purpose:
Working with our existing clinicians to support the work of the GPs in the practice with care-home visits and ward-rounds, housebound patients and acute home visiting.
Weekly care home rounds, liaising closely with care home staff, practice GPs and colleagues in support of their residents.
Support the GPs by triaging and attending acute home visits in rotation with the established clinical team. To manage acute illnesses in patients who are not able to attend the surgery, may include arranging admissions to hospital or liaising with community teams, palliative care teams and social care.
Delivering support to our housebound patients. To improve management of LTCs, help with decisions around ceilings of treatment and reduce hospital admissions.
To deliver a high standard of patient care, using advanced autonomous clinical skills, and a broad in-depth theoretical knowledge base.
To contribute to the QOF/IIF targets for the Practices and support the ongoing development of both the care home and visiting service.
About us
Wyvern Health is a Primary Care Network (PCN) of three GP practices, Priory Road Medical Centre, Lawn Medical Centre and Old Town Surgery.
We are high performing practices with a strong ethos of continuity of patient care, teamwork and peer support. We have a list size of circa 27,500 patients, our clinical system is SystmOne and our most recent CQC rating was GOOD.
The PCN has 10 GP partners and an extensive clinical team of advanced nurse practitioners, general nurses, pharmacists, pharmacy technicians, first contact physiotherapists, care co-ordinators, health care assistants and admin support staff, working alongside a management team and sharing best practice to benefit both our teams & our patients alike.

To include ward rounds at nursing/care homes to deal with clinical queries some which may be urgent to avoid admission, medication reviews, referrals and end of life care.
Manage requests from patients requiring acute home visits. Triaging appointments as well as visiting.
Support in the care of housebound patients.
Ensure the highest standards of treatment and care for all patients (including complex patients, both acute and chronic). This will involve diagnosis, implementation of care plans to ensure physiological and psychological factors are taken into consideration and ongoing evaluation of any plans to ensure patient compliance.
Working closely with our team of Pharmacists to prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
Health promotion and motivation of patients to improve general health.
Communicate effectively with patients and carers/care home staff (nurses/carers), recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background, and preferred ways of communicating.
Recognising issues and gathering sufficient information to refer (e.g. drugs; domestic violence; child protection; vulnerable adults; senior patients; social problems).
Manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients.
Be mindful of work areas and practices to ensure they are safe and free from hazards and conform to health, safety and security legislation, policies, procedures and guidelines.
Follow all necessary infection-control measures according to local and national guidelines.
