An exciting
opportunity has arisen at Priory Road Medical Centre for an enthusiastic
Practice Nurse to join our team starting on 1 November 2024.
We are a

Hours per
week 18 over 3 days
The role
will be varied and will include chronic disease management, wound care,
treatment room work, national screening work such as cervical cytology and the
national immunisation and vaccination schedule, providing travel health advice
and administering appropriate vaccinations and health promotion to help educate
patients.
Please note
if you or any immediate family members are registered at the practice, all
parties will be required to change to an alternative GP provider upon an offer
of employment.

About us
This is an
excellent opportunity for an enthusiastic, experienced, positive and flexible practice nurse to be part of the Priory Road Medical Centre team
Based in
Swindon, this friendly busy surgery has a patient list of approximately 8700
patients
Wyvern
Health Partnership is a newly formed partnership of five GP practices, formed
in 2018 to improve the quality of care for over 50,000 patients across the
area.
The
partnership incorporates Hawthorn Medical Centre, Lawn Medical Centre,
Merchiston Surgery, Old Town Surgery and Priory Road Medical Centre.
The
practices came together in response to the increased demand for primary care
services in Swindon and the considerable pressure this put on resources, and on
staff in particular.
The partnership
has 23 partners and a large team of GPs, nurse practitioners, general nurses,
health care assistants and support staff, working together and sharing best
practice to benefit both our teams & our patients alike.

Job responsibilities
To manage the care of patients with long term
conditions by assessing, planning, implementing, and evaluating treatment plans
for patients with long term conditions (or at risk of developing one) that meet
identified needs and promote health and well-being.
Provide the full range of nursing services as
required to meet the needs if the practice, including venepuncture, ECG
measurement, blood pressure measurement, urinalysis, immunisations (children
and adults including travel), injections, cervical smears, swabs, travel
services, assessment of risk factors, lifestyle advice, weight management, ear
care, suture removal, wound care, doppler assessments, family planning, etc.
Educate and support patients as needed,
including adopting healthy lifestyles, appropriate self-care
To assess the needs of patients, providing
appropriate care/treatment in conjunction with other health care professionals,
or independently, as appropriate.
Refer to colleagues or other agencies as
appropriate.
Prioritise
health problems and intervene appropriately to assist the patient in complex,
urgent or emergency situations, including initiation of effective emergency
care.
Support
initiatives, including contacting patients, to improve uptake of screening,
vaccinations or other services and support the achievement of targets (e.g.
under the Quality and Outcomes Framework).
To maintain
accurate and contemporaneous records to relevant professional
standards/guidelines.
To ensure
awareness of statutory and local child and adult protection procedures,
including systems of referral. Ability to recognise signs and symptoms of child
abuse.
Administration
and professional responsibilities
Participate in the administrative and
professional responsibilities of the practice team
Ensure accurate and legible notes of all
consultations and treatments are recorded in the patients notes
Ensure the clinical computer system is kept
up to date, with accurate details recorded and amended
Ensure appropriate items of service claims
are made accurately, reporting any problems to the practice administrator
Ensure accurate completion of all necessary
documentation associated with patient health care and registration with the
practice
Ensure collection and maintenance of
statistical information required for regular and ad hoc reports and audit
Attend and participate in practice meetings
as required
Restocking and maintenance of clinical areas
and consulting rooms
To assist the doctor to carry out minor
operations as required.

Training
and personal development
Training requirements will be monitored by
yearly appraisal and will be in accordance with practice requirements. Personal
development will be encouraged and supported by the practice. It is the
individuals responsibility to remain up to date with recent developments.

students of all disciplines and the introduction of all members of the practice
staff where appropriate
Maintain continued education by attendance at
courses and study days as deemed useful or necessary for professional
development, ensuring PREP requirements are met.
If it is necessary to expand the role to
include additional responsibilities, full training will be given.
