We are seeking a motivated and enthusiastic Advanced Clinical Practitioner to join our welcoming and supportive team at our well-established practice in Cheslyn Hay. We are a friendly, patient-focused surgery with a strong team ethos and excellent working relationships across our multidisciplinary team, which includes 2 GP Partners, 1 Advanced Nurse Practitioner, 2 Practice Nurses, 1 Healthcare Assistant, 1 Phlebotomist, and a large, dedicated administrative team. Main duties of the job

The Advanced Clinical Practitioner will deliver high-quality, patient-centred care through assessment, diagnosis, and management of acute and minor illness presentations. The role includes running minor illness clinics, managing long-term conditions, prescribing independently, developing treatment plans, and coordinating referrals. You will work both autonomously and as part of a supportive multidisciplinary team, contributing to service development, quality improvement, and excellent patient outcomes.

Main Duties

  • Assess, diagnose, and manage patients with acute and minor illnesses.

  • Run minor illness clinics and manage long-term conditions.

  • Administer seasonal and COVID vaccinations in collaboration with the PCN.

Essential Qualifications

  • Current NMC or HCPC registration.

Desirable Qualifications

  • Postgraduate qualifications relevant to chronic disease management, minor illness, or urgent care.
