Job summary
Railway Medical Group are looking for a Lead Practice Nurse
to join our friendly and dynamic team in Blyth, Northumberland.
We are an expanding GP Practice with over 27,300 patients in
Northumberland and are dedicated to improving the health and wellbeing of our
diverse patient community.
We are looking for a motivated team player who can
demonstrate a high standard of clinical excellence and knowledge to provide a
safe, effective and easily accessible range of health services.
Main duties of the job
To manage and provide clinical supervision and guidance to
nursing staff and Healthcare teams.
Assessing patient needs and developing
individualised care plans. Administering medication and treatments as
prescribed.
Monitoring patient progress and making necessary adjustments to
care plans.
Facilitating communication between staff, team collaboration.
To assist in and perform routine tasks related to patient
care as directed by nursing staff and GPs.
Chronic Disease Management to include: Diabetes, Asthma,
COPD, Chronic Heart Disease, Hypertension, to develop and implement care plans
for patients with Long term conditions, promoting self-care.
Cytology, Childhood immunisations, NHS Travel vaccinations,
phlebotomy, womens health, injections
Participate in areas within QOF and other local initiatives
in order to enhance patient care
Be involved in Safeguarding, Palliative Care meetings,
clinical audits
Be involved in clinical practices and policies, ensuring
national policies are in place and shared with the wider clinical team.
To keep accurate clinical detailed health records on Emis.
Undertake mandatory and statutory training
requirements and promote the role of Practice Nurse in the provision of care

Job responsibilities
To manage and provide clinical supervision and guidance to
nursing staff and Healthcare teams. Assessing patient needs and developing
individualised care plans. Administering medication and treatments as
prescribed. Monitoring patient progress and making necessary adjustments to

Chronic Disease management including data recording and
patient education for asthma/COPD, cardiovascular disease and diabetes
Management of diabetic patients including regular reviews,
annual foot checks, patient education and liaison with other members of the
healthcare team e.g. GPs, dietician. Experience in this area would be desirable
but not essential as appropriate training can be given
NHS travel medicine
Immunisation of adults and children
Venepuncture
Cervical screening and swabs
Contraception and sexual health advice
Requesting pathology tests, for urine culture, swabs,
routine blood tests etc.
Managing, planning and delivering to contractual targets and
alerts such as QOF
Taking a lead responsibility for a chronic disease
condition.
Developing Nursing services within the practice.
Maintaining and organisation of dedicated QOF registers.
Maintaining and cleaning equipment used within the practice.
Maintaining GP and Nurse rooms, stocking and rotating items
as required.
Chaperoning and assisting patients who are being examined by
another clinician.
Maintaining general tidiness and cleanliness of nurses and
treatment rooms.
Maintaining stock of leaflets as instructed by GPs and
Practice Manager.
Ordering Health Promotional Literature.
Ordering stock as needed including vaccinations to maintain
stock levels.
Participating in the administrative systems in the Practice.
Maintaining accurate records at all times.
A duty to advise the Lead GP of potential problems or
errors.
Attend and participate in any Practice Meetings when
appropriate.
Any other delegated duties as appropriate to the post.
