Peartree Group Practice is a forward-thinking,
patient-focused GP practice serving a diverse community in Hertfordshire. We
are committed to delivering high-quality, proactive, and holistic care. Our
friendly and multidisciplinary team includes GPs, nurses, HCAs, care
coordinators, and other allied health professionals. We have a strong medicines
management team, a lead GP, Pharmacist, Pharmacist technician and trained
administrators who support as prescriptions clerks. We are now looking to
welcome a motivated and knowledgeable Clinical Pharmacist to join our
clinical team and enhance patient care.The Role
As a Clinical Pharmacist, you will play
a key role in the delivery of safe, effective, and evidence-based medicines
management. You will work closely with GPs and the wider practice team to
optimise prescribing, conduct structured medication reviews, and manage
long-term conditions.
Main duties of the job
Key Responsibilities:
- Undertake structured medication reviews (SMRs) for patients with long-term conditions, polypharmacy, or those in care homes.
- Support medicines reconciliation post-discharge.
- Assist with repeat prescription authorisation and medication queries.
- Provide expert advice on clinical medicines management to colleagues and patients.
- Support the practice in achieving QOF and IIF targets related to prescribing.
- Audit prescribing trends and support quality improvement initiatives.
- Contribute to chronic disease management (e.g. hypertension, asthma, diabetes) as appropriate.
- Work closely with our medicines management lead GP and Practice Manager to ensure safe prescribing
- Providing education to colleagues and patients regarding prescribing and medicines not requiring prescribing

- Excellent communication and interpersonal skills.
- Ability to work both independently and as part of a multidisciplinary team
- Strong clinical knowledge and attention to detail.
- Understanding of primary care and the evolving role of pharmacists within GP surgeries
Desirable
- Desirable:
- Independent Prescriber qualification or willingness to work towards
- Experience working in primary care, PCNs, or GP surgeries.
- Familiarity with System One
- Experience in LTC such as Diabetes, Hypertension, Asthma
