The post is part time 30 hrs per week . You will be working for Drs Gilchrist and Fulford at Gibb Lane , Sudbury , Ashbourne , Derbyshire , DE6 5HY and Tutbury Health Centre , Monk St, Tutbury , Burton on Trent , Staffs , DE13 9NA.
Hours 30 hours per week. Working hours to be discussed but a flexible approach is required. You will be based at the Health Centre in Tutbury but will be expected to work at the Sudbury Surgery as and when required. The practice hours of opening are from 8 am to 6 pm.
Job summary
The post holder is a highly motivated and capable experienced nurse practitioner who is a team player and who will act within their professional boundaries. They will provide a high standard of independent , autonomous , effective and safe care for patients presenting acutely from initial history taking, clinical assessment, diagnosis and decision making , treatment , follow up and evaluation of care. They will make necessary referrals for investigation in the appropriate manner. They may be required to undertake telephone triage. They may see extra and emergency patients as required. They may also be asked to review incoming pathology reports and mail. They will communicate and work collaboratively with the whole practice team to meet the needs of patients, and also supporting the delivery of policy and procedures. They will be prescribers working within the boundaries of their clinical knowledge and experience. In order to work at this level, NMC requirements for advanced practice must be met
Key responsibilities
Clinical practice
- Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
- Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
- Instigate necessary invasive and non invasive diagnostic tests or investigations and interpret the results of these
- Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs
- Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
- Diagnose and manage both acute conditions and acute exacerbations of chronic conditions within scope of practice, integrating both drug- and non-drug-based treatment methods into a management plan
- Refer patients to other services or agencies as appropriate both within and outside the practice.
- Prescribe safely and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice
- Work with patients in order to support compliance with and adherence to prescribed treatments Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
- Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health, which will include referring to GP for assessment and management where appropriate.
- Contribute to the practice achieving its quality targets
- Be aware of and contribute to the planning and delivery of practice based commissioning
- Produce accurate , contemporaneous and complete records of pt consultations , consistent with legislation , policies and procedures , and record appropriately in order to facilitate the practices delivery of Q.O.F and Enhanced Services
Communication
- Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment
- Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating
- Anticipate barriers to communication and take action to improve communication
- Act as an advocate for patients and colleagues
- Ensure awareness of sources of support and guidance (eg PALS) and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate.
- Communicate effectively with other team members
- Recognise peoples needs for alternative methods of communication and respond accordingly
Delivering a quality service
Recognise and work within own competence and professional code of conduct as regulated by the NMC
Prioritise, organise and manage own workload in a manner that maintains and promotes quality
Deliver care according to NICE guidelines and evidence-based care
Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation
Initiate and participate in the maintenance of quality governance systems and processes across the organisation and its activities
Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required
In partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate
Evaluate patients response to health care provision and the effectiveness of care
Support and participate in shared learning across the practice and wider organisation
Use a structured framework (eg root-cause analysis) to manage, review and identify learning from patient complaints, clinical incidents and near-miss events
Assess the impact of policy implementation on care delivery
Contribute to the appraisal of the performance of the team, providing feedback as appropriate
Understand and apply legal issues that support the identification of vulnerable and abused children and adults, and be aware of statutory child/vulnerable patients health procedures and local guidance
Leadership personal and people development
- Take responsibility for own learning and performance including participating in clinical supervision and acting as a positive role model
- Support staff development in order to maximise potential
- Training requirements will be monitored by annual appraisal and will align with practice requirements. Personal development will be encouraged and supported by the practice. It is the individuals responsibility for personal and professional development and to remain up to date with recent developments
- Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice
- Critically evaluate and review innovations and developments that are relevant to the area of work
- Enlist support and influence stakeholders and decision-makers in order to bring about new developments in the provision of services
- Contribute to the development of practice guidelines, protocols and standards
- Promote the role of the advanced nurse practitioner in general practice
- Participate in the introduction of all members of staff
- If it is necessary to expand the role to include additional responsibilities , additional training will be given
- Understand own role and scope and identify how this may develop over time

Management of risk
- Manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients
- Apply infection-control measures within the practice according to local and national guidelines
