Job summary

We are looking for a Prescription Clerk to join our professional and friendly administration team based in Gloucester. This is an opportunity to join a well-led, well-run and mutually supportive team and play your part in helping primary care patients. This role will be an afternoon cover position to support the team during the later part of the day.

Main duties of the job

The role is to work with and support the pharmacist team to provide a safe and effective prescription service, including the handling of prescription enquiries from patients and external sources.

Job description

Job responsibilities

Understand the electronic prescribing system within the practice.

Process repeat prescriptions on a daily basis, ensuring compliance with the practice protocols.

Handling weekly prescriptions for vulnerable patients and dealing with the queries related to these.

Support the reception team with medication requests using surgery protocol to signpost to appropriate answer.

Ensure all prescriptions are signed by a GP, ready for collection within 48 hours from time of request and ready for issue by the patients pharmacies.

Ensure that problems and queries regarding repeat prescriptions are brought to the attention of the pharmacist and that follow up action is taken to resolve such matters

Perform regular searches on drugs requiring monitoring, hence minimising the risk of adverse effects. Ensure all current medication has relevant, current authorisations.

Ensure that medication reviews have been undertaken as required and flag for action by a prescriber if overdue.

Assist with the coding of medication reviews.

Promote cost-effective prescribing at the practice through switching drugs to the generic preparation when appropriate and encouraging the use of local primary care trust formulary medication choices.

Deal with prescription queries from a range of healthcare professionals including; GPs, nurse prescribers, hospital outpatients/consultants, community pharmacists, community nursing staff, carers, care home etc, as well as reception/administration staff at the practice.

Deal with medication changes from all hospital letters, removing old medication adding new or making dose changes with GPs agreement and oversight documented in patient notes. Informing patients of changes where necessary and any review monitoring requirements.

Manage prescriptions for patients prescribed weekly medication in compliance aids

Frequent, prolonged VDU use
