Job summary

Westbourne Medical Centre are currently recruiting for a Clinical Pharmacist (ARRS funded role) to help deliver our vision of high-quality medical services at our well-established, progressive, friendly practice team in North Ormesby for 3 to 4 days per week.

The clinical teams are also will supported by an excellent administration and management team; this then allows all in the clinical team to concentrate on their clinical work.

Main duties of the job

Work closely with the GP's/ NP and Nurse to resolve day to day medicine issues.

Carry out telephone appointments with patients regarding medication management.

To work as part of the multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.

Develop a relationship with other pharmacy professionals across the PCN.

Be a prescriber or completing training to become a prescriber.

Be responsible for the care management of patients with long term chronic diseases and undertake clinical reviews ensuring they get the best use of their medicines and making the appropriate recommendations to GPs for medicine improvement.

Answer relevant medicine related enquiries from GP's, other practice staff, other healthcare professionals and patients.

Manage medicines from hospital discharges, intermediate care and into care homes including identifying and rectifying unexplained changes and ensuring patients receive the medicines needed post discharge.

Contribute to the achievement of QOF.

Undertake clinical audits when required.

Action acute prescription requests.

Carry out medicine optimisations and medicine prescriptions.

Job description

Job responsibilities

Carry out telephone appointments with patients regarding medication management.

To work as part of the multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.

Develop a relationship with other pharmacy professionals across the PCN.

Be a prescriber or completing training to become a prescriber.

Be responsible for the care management of patients with long term chronic diseases and undertake clinical reviews ensuring they get the best use of their medicines and making the appropriate recommendations to GPs for medicine improvement.

Proactively manage patients with complex polypharmacy such as older patients, people in care homes, those with multiple co-morbidities and people with learning difficulties including autism undertaking clinical medication reviews and producing recommendations to GPs on prescribing and monitoring.

Provide specialist expertise in medicines whilst addressing public health and social care needs of patients, tackling inequalities.

Provide leadership on person centred medicines optimisation and quality improvement, contributing to QOF and enhanced services.

Managing minor and common ailments under levels of competence, signposting to pharmacy, or referring to GP when appropriate

Promote selfcare to improve medicine wastage.

Answer relevant medicine related enquiries from GP's, other practice staff, other healthcare professionals and patients.

Manage medicines from hospital discharges, intermediate care and into care homes including identifying and rectifying unexplained changes and ensuring patients receive the medicines needed post discharge.

Take a central role in the clinical aspects of shared care protocols, liaise with specialist and community pharmacists.

Take a leadership role in the integration of general practice with the wider healthcare teams to help improve patient outcome ensuring better access to healthcare.

  • Minimum of 2 years post qualification experience
  • Excellent interpersonal skills.
  • Excellent written and verbal communication skills.
  • Can plan, manage, monitor, advise and review general medicine optimisation in core areas for long term conditions.
  • Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate.

Desirable

  • Able to work under pressure and to meet deadlines and produce timely and informative reports.
  • In depth therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare.
