Job summary

Reading Holybrook Primary Care Network has exciting opportunities for hardworking and enthusiastic Paramedic Practitioners to join our PCN.

Please note, you may be required to travel between sites.

Salary will be based on experience and skills according to the PCNs Competency Framework. Salary ranges are from £38,000-55,000 WTE (depending on experience).

A full-time candidate will work 37.5 per week. Monday to Friday between 0800-1830 (including a one-hour unpaid break daily)- however part time opportunities will also be considered.

Main duties of the job

Our PCN role(s) will offer you the opportunity to build on your experience of working effectively within a multi-disciplinary, healthcare team, as you will work alongside fellow professionals within the PCN in support of one another.

We are therefore looking for dynamic, primary-care focused colleagues to join our team.

The main duties include:

  • managing acute and chronic conditions, providing subject matter expert advice
  • identify and support patients at risk of developing long term conditions
  • provide advanced specialist clinical care in accordance with clinical based evidence, NICE and NSF.

About us

Reading Holybrook Primary Care Network, based in South Reading is a small, innovative PCN of 3 practices, with one of our member practices being renowned for their excellent teaching reputation. The practices are as follows:

  • Milman Road and Kennet Surgery
  • Broad Street Health Centre
  • Abbey Medical Centre

We have progressed well with the national recruitment agenda and have already recruited clinical pharmacists, social prescribers, paramedics, physicians associates, and advanced clinicians. We have a training programme for staff that is second to none, and our ethos is for all staff to work to the top of their licence.

Clinical colleagues are required to provide primary care focused holistic care in accordance with the General Practice Contract and inline with their scope of practice.

This includes helping to manage same day acute illnesses as well as long term condition management. We expect our clinical team members to support others to always develop and embed health promotion and wellbeing.

Whilst clinical team members work autonomously, they have dedicated support and supervision with scheduled de-brief (and an allocated supervisor). As an Independent Prescriber, or working towards this, our expectation is that you will adhere to best practice and guidance, ensuring you able to support patients with prescribed and over the counter medicines. You will contribute to the practice achieving local and national targets.

You will confidently liaise with external agencies to ensure patients are supported appropriately and support the clinical team with all safeguarding matters in accordance with local and national policies.

Qualifications

Essential

  • Degree in Paramedic Science or equivalent.
  • Professional registration with HCPC and no current issues under investigation
  • CoP Dip. Primary & Urgent Care Exam
  • Minor illness adult and child
  • Driving Licence
  • Level 3 Safeguarding (Children)

Desirable

  • Master's Degree or valid credentialling certification required to be considered for Advanced Level Practitioner
  • Long term condition speciality (e.g., Diabetes, Asthma, COPD)
  • Independent Non-Medical Prescribing
  • Mentoring
  • Advanced Life Support
  • Advanced Paediatric Life Support
  • Palliative Care
  • Familiarity with EMIS AND Adastra systems
  • Experience of conducting audits: examining clinical notes, RCGP Toolkit, meeting national targets (QOF, Prescribing, IIF), interpreting data, feeding back results
