Job summary

Fenland Group Practice is looking for a part time Care Navigator to come and work at our Ramsey Branch

There will be the option to cover shifts at our other sites as well to cover holiday and sickness

Main duties of the job

- dealing with queries from patients both in person and face to face

- obtaining information via our e-consultation system

- understanding the services in the area in order to navigate patients the the most appropriate help

- understanding of confidentiality

- booking appointments for patients

- contacting patients to pass on messages from the GP/Clinical team

Fenland Group practice is based in the Fens, over 4 sites. It is a very busy practice with just over 13,000 patients.
