Job summary

We have an exciting new opportunity to recruit a permanent Mental Health Practitioner, to work in Hawthorn Medical Centre. Successful candidates must be experienced Mental Health practitioners and have experience of working in a Primary Care and/or Community Mental Health Team setting.

This is an exciting role for those who are proactive and passionate about innovation and providing high-quality care to people who have been identified as in need of mental health support in a primary care setting. The Practitioner will provide triage, assessment, interventions and lead on development and implementation of specialist mental health training in the practice, whilst also working alongside a multi-disciplinary team including Physiotherapists, Pharmacists, a trainee Social Prescriber, Home Visiting Clinician, a strong multi skilled Nursing Team and Salaried GP's.

Main duties of the job

This role will work in Hawthorn Medical Practice, to provide early assessment, treatment and improved access to appropriate care pathways and provide evidence-based interventions for people with mental illness.

This post is full-time 37.5 hours, however consideration will also be given to a job share.

Undertake a comprehensive assessment of a patients mental health needs.

Assess and manage risk to patients and others.

Essential

  • ESSENTIAL KNOWLEDGE, SKILLS AND EXPERIENCE
  • To be a fully qualified mental health nurse.
  • Current registration with the Nurse and Midwifery Council and commitment to ongoing Continued Professional Development.
  • To be a non-medical prescriber or prepared to study to obtain the qualification.
  • To have experience of practising in a wide variety of settings, understanding the role of primary and secondary health services in managing patients with mental health problems.
  • Front line experience of using advanced assessment skills to determine the diagnosis and severity of mental illness.
  • Extensive experience in assessing risk and developing risk management strategies.
  • Vast knowledge of the different treatment approaches and rights of people with mental health problems.
  • To have a deep understanding of the impact on mental illness on the individual, their carer and their family.
  • Exceptional verbal communication skills, being able to engage with patients of all backgrounds and being adept at recognising when a more assertive approach is needed.
  • Outstanding active communication skills showing empathy, picking up on cues and being able to check a patients understanding.
  • To have a sound understanding of the relevant legal frameworks including Mental Health Act, Mental Capacity Act, Safeguarding and the CPA.
  • To be highly competent at written documentation.
  • Proficient IT skills with flexibility to learn new computer systems.
  • Substantial experience in education of other health care professions.
  • This could be a developing role if the post holder does not have all of the skills required. In these circumstances support and training will be provided.
