Job summary

Mill Bank Surgery is looking for an experienced Advanced Nurse Practitioner to join our busy, well established and high achieving town centre practice, working alongside our existing supportive friendly clinical and administrative teams.

The position is permanent, offering (5 hours per week) days to be agreed.

Salary dependant on experience.

Main duties of the job

You will be an Independent Prescriber with experience in assessing, diagnosing and implementing a range of care interventions for patients.

About us

Our clinical team consists of 3 GP partners, 3 salaried GPs, 2 advanced nurse practitioners, 2 practice nurses, and 1 healthcare assistants. We also have PCN additional roles including Clinical Pharmacists, 2 Pharmacy Technicians, 3 First Contact Physiotherapists, a Social Prescriber and a Mental Health Practitioner. We are supported by a resilient and longstanding administrative team.

The practice has a list size of 10,500 patients and the post holder will already be an independent prescriber, required to work as part of our clinical team on a daily basis providing assessment, diagnosis and treatment at first point of contact in the practice, in patients homes, or in a Care Home setting. JOB SUMMARY

To practice as an Advanced Nurse Practitioner undertaking the assessment, evaluation and planning of patient care, providing clinical treatments for patients in a primary care setting.

KEY RESPONSIBILITIES

Clinical

Providing assessment, diagnosis and treatment at first point of contact by attending to patients according to patients needs either in the Practice, Care Home setting or patients homes.

Working in the Same Day Team diagnosing and treating patients with minor illness and other urgent presentations face-to-face or via telephone.

Accurately triaging and prioritising patients, including in acute situations, demonstrating the use of a variety of techniques to elicit the history of an event/illness, including past medical and drug history.

Evaluating clinical information from examination and history taking, initiating appropriate treatment and/or referral and requesting and interpreting tests eg., blood tests, imaging and ECGs to aid the diagnosis of complex conditions.

Using broad clinical guidelines (NICE etc) relating them to individual cases, providing assistance and feedback to the organisation and others on how these should be interpreted.

Signposting appropriately to other services in the identified healthcare pathway.

Prescribing medications as an independent prescriber.

Referring patients to appropriate care settings, within locally agreed healthcare pathways.

Maintaining accurate and systematic documentation of patient interactions and clinical care provided, including adverse events.

Advising and supporting members of the public on healthy lifestyles, health promotion and disease prevention.

Providing clinical advice to other members of the team where appropriate.

Communication

Liaising and interacting with multi-professional teams across organisational boundaries, including primary care, social services and secondary care providers according to patient needs and appropriate care pathways.

Maintaining communication with all healthcare professionals and outside agencies to ensure seamless, patient-centred service provision.

Ensuring effective communication so that the patient receives the appropriate level of care and wherever possible avoiding unnecessary hospital admission.

Undertaking clinical audits in line with the organisations clinical audit plan, and to participating as required in equipment testing and other research.

Contributing to the analysis of information to inform future development of the service.

Identifying risks to service provision, and highlighting them to the Practice Partners

Working within the requirements of Data Protection legislation as it applies to patient records.
