We are seeking a dedicated and experienced Advanced Nurse Practitioner (ANP) to join our team. This is a part-time role across three consistent weekdays (Monday, Wednesday, and Friday), supporting high-quality patient care in a warm, community-focused family GP practice.
The post-holder will be employed by the Primary Care Network (PCN) but will be based entirely and work specifically at North Beverley medical centre becoming an integral part of our on-site clinical team.

Main duties of the job
Clinical assessment
To provide advanced assessment skills in history taking and clinical examination working autonomously to formulate a diagnosis and initiate an appropriate plan of care, including referral(s) to other health and social care professionals as necessary.
Prescribe appropriate medication and preparations within existing legislative framework for nurse prescribing in relation to evidence based practice and taking into account the financial implications of cost effective prescribing.
Acute and Chronic disease management
Work with GPs and all members of the practice health team in developing protocols for diagnosing and managing minor illnesses and the management of chronic diseases.
Work with the nursing, clinical pharmacy and medical team to provide proactive management of patients with chronic diseases in accordance with NSF and local requirements.
Work with members of the practice healthcare team to promote integrated seamless care for patients and carers.
Plan and organise a broad range of complex activities. Formulating long term plans for managing chronic disease, in line with local and national standards and targets.
Independently assess, diagnose, and prescribe (V300 essential)
Offer a mix of face-to-face, telephone, and online consultations
Contribute to QOF, long-term condition reviews, and preventative healthcare
Collaborate closely with GPs, nurses, HCAs, and support staff
Use SystmOne for clinical documentation and patient care management

- Minimum of 5 years post registration experience in general practice or relevant area
- Evidence of appropriate continuing professional development activity to maintain up-to-date knowledge and on-going competence in all aspects of NP role
- Experience of Health Promotion
- Experience in management of long term conditions eg. Asthma, COPD, diabetes, CHD
- Experience of managing minor/acute illnesses
- Evidence of working autonomously and as part of a team
- Proven ability to evaluate the safety and effectiveness of their own clinical practice
- Use of clinical computer systems

- Registered first level nurse
- Relevant nursing degree
- Masters degree or equivalent post-registration education to meet the RCN recommendations for Advanced Practice
