Job summary
Beverley PCN are looking for an Advanced Nurse Practitioner for 24-37 hours per week to work in a long-established, successful surgery . We have excellent supportive nursing team, who work together to provide high quality patient care. You will be based at North Beverley Medical Centre.
The Practice aims to provide patient continuity to achieve high-quality care outcomes. Actively involved with the PCN to provide enhanced integrated services within the community.
They will be responsible for attending to patients who wish to be reviewed by a health care professional; diagnosing and treating patients presenting with a variety of illnesses, seeing and advising people in respect of their continuing medical and nursing needs. These patients will be a combination of telephone triage calls and booked appointments. The nurses will need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner. The nurses will be responsible for advanced care planning.
Main duties of the job
Assess, plan, provide and evaluate nursing care to meet the care needs of individuals in the practice population. Assess, examine, diagnose and treat specific conditions in accordance with agreed medical/ nursing protocols, and within own area of competence or speciality. Provide access to GPs as necessary. Undertake diagnostic, health screening and health promotion at consultation/ opportunistically. Triage and treat patients wishing to see a health care professional making any necessary referrals to other members of the primary health care team Provide specific evidence based practical skills and knowledge in telephone and clinics. Ensure clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations. Advise patients on general health care and minor ailments with referral to other members of the primary and secondary health care team as necessary Undertake the collection of pathological specimens including intravenous blood samples, swabs etc. Perform investigatory procedures requested by the GPs Report on and action results
About us
We have a growing list size of 7200 patients, are a friendly, well organised practice using EMIS Web/Docman 10. We
are an integral part of the Beverley Primary Care Network. The clinical team
consists of two GP partners, two salaried GPs, an Advanced Nurse Practitioners
and a highly experienced Practice Nursing team.
We have an administrative team of 10 who support our services.
Our ethos is very patient centred, and
we aim to continue to proactively evolve the practice from our family practice
roots to meet future challenges together with the growing and changing needs of

- Masters degree or equivalent post-registration education to meet the RCN recommendations for Advanced Practice
