An exciting opportunity has arisen for a Care Coordinator to join our friendly and supportive team at Verwood Surgery. This is a rewarding and varied role combining administrative responsibilities with direct patient contact and may include phlebotomy and Healthcare Assistant duties.
Key responsibilities include (but are not limited to):
- Getting involved in practice projects such as Care Navigation and directing patients to the correct help
- Administrative support document workflow, prescriptions, managing tasks and emails
- Supporting the Patient Participation Group, including organising agendas, minutes, and associated administration
- Assisting with Covid and Flu clinics when required
- Contributing to the practice newsletter and updating the waiting room media screen
- Potential for undertaking phlebotomy and associated work-ups
About you:
- You will have a strong administrative background and a keen interest in healthcare
- Excellent attention to detail and the ability to respond sensitively to patient needs are essential
- You should be organised, adaptable, and comfortable working as part of a multidisciplinary team
Main duties of the job
The role of Care Coordinator at the Practice encompasses a
range of patient facing and admin duties. This includes answering phone calls
to the practice and queries, making phone calls and sending text
messages/emails/letters, and completing all other admin duties required to
provide our patients with the best care possible, with the long-term aim to
also provide patients with phlebotomy and basic healthcare assistant duties.
Working alongside the wider team supporting continuity of
care, signposting, providing links to stakeholders by being their nominated
first point of contact, the role aims to provide more seamless and integrated
care.
General computer skills are essential, experience of SystmOne
would be an advantage. Successful candidates who have been selected for

We are an exceptionally friendly and supportive practice
with high rates of staff retention.
The practice team is made up of fourPartners, three Salaried
GPs, one Retainer GP, three practice nurses, Physicians Assistant,
Phlebotomists, Frailty Team, management team and our admin staff. We are a
training practice and regularly have a Registrar and a Foundation Year Doctor.
Verwood surgery is a modern purpose-built medical centre in
the heart of our community. We have a minor ops suite as well as an onsite
pharmacy, physio practice and veterinary practice and are adjacent to the
Verwood Hub community centre.
We have a good CQC rating and are part of Crane Valley PCN
and Castleman Federation and enjoy working closely with them in our locality.
We have a PCN Pharmacy team, Social Prescribers, Health Coach, Mental Health
Practitioner, Community HCA and Care Home Team. We also have onsite Health
Visitors and Counselling.
