Digital and Transformation Lead PCN

Job summary

Bradford City 5 PCN is seeking a digitally skilled and data-driven professional to lead on the development of local digital transformation initiatives, with a strong focus on data quality, insight generation, and population health analytics.

This position is 10hrs per week.

Pay is aligned with ARRS funding banding (Pro rata)

Main duties of the job

This role is aligned with the PCN DES 2025/26 requirements and will directly contribute to our core digital offer, access improvement, and health inequalities reduction targets. The post holder will work collaboratively with PCN practices to extract, analyse and interpret data, create SystmOne reports, build custom dashboards, and support the wider team to use data effectively in both strategic and operational settings.

Bradford City 5 Primary Care Network (PCN5) is a network of 9 GP practices comprising of around 48,000 patients. The PCN covers the geographical area embracing Bradford Moor, Thornbury and Eccleshill

Job responsibilities

Key Responsibilities.

Data Quality and Analytics

Design and maintain SystemOne searches and reports for clinical and operational metrics.

Extract, clean and interpret data to support the PCN's health inequalities work, IIF targets, and DES-aligned priorities.

Develop interactive dashboards (e.g., Power BI or Excel) for visualising appointment activity, workforce utilisation, and patient outcomes.

Identify data inconsistencies, support data cleansing, and help practices improve coding and data quality standards.

Track trends and highlight areas of variation or improvement across practices.

Digital Transformation Support

Assist practices in maximising use of existing digital tools including NHS App features, online consultations, and appointment mapping systems.

Provide digital input into the PCNs transformation programmes and contribute to local digital strategy development in line with ICS direction.

Support the PCN in meeting DES digital transparency and access requirements, including timely submission of data to NHS England.

Practice Support and Upskilling

Act as a point of contact for digital and data queries across practices.

Provide coaching and troubleshooting support for admin and clinical staff in using digital systems (e.g. SystemOne templates, searches, reports).

Promote digital inclusion and support practices to identify and reach digitally excluded patient groups.

Monitoring and Evaluation

Regularly report on performance indicators linked to the Investment and Impact Fund (IIF), patient access metrics, and local inequalities targets.

Monitor uptake and use of digital tools, evaluate the impact of service changes, and provide feedback to the leadership team.

Contribute to audit cycles and quality improvement projects using data insights.

Experience

  • Experience working in general practice or primary care networks.
  • Strong proficiency in SystemOne search/report builder and Microsoft Excel.
  • Experience with creating or managing dashboards Power BI desirable.
  • Knowledge of NHS data flows, coding standards e.g. SNOMED, and data governance.
  • Ability to interpret and clearly present complex data to a non-technical audience.
  • Experience supporting staff across multiple sites or teams.
  • Strong communication and collaboration skills.
  • A commitment to improving care through digital innovation.

Desirable
