Job summary
The Bradford City 5 Primary
Care Network (PCN) is seeking a dynamic and experienced PCN Manager to support
the delivery of high-quality, equitable, and innovative services across its
nine practices. This new part-time role will enhance the resilience of the PCN
leadership structure by working closely with the Clinical Director and Senior
PCN Manager to ensure smooth day-to-day operations and support the PCNs
strategic priorities.
The PCN Manager will form an integral
part of the PCN leadership team working alongside the Clinical Director,
management team and the PCN board members. The PCN Manager will act as a point
of expertise, coordinate the successful achievement of all governance
requirements, translate national and local policy into meaningful changes, and
will support practices to achieve improved outcomes for the local population.
They will also along with the management team liaise with the PCN Managers and
Clinical Directors of other PCNs.
This is a 10hr a week position, temporary contract for 6-12 months

1. Operational Support & Programme Delivery
- Provide high-level project and programme management support across PCN initiatives.
- Assist with the coordination and delivery of Enhanced Services, IIF, and PCN DES priorities.
- Monitor progress and outcomes of workforce initiatives and population health projects.
- Support the roll-out of digital, access, and health inequalities programmes in line with Core20PLUS5 and NHS planning guidance.
2. Communication & Engagement
- Act as a liaison between member practices and the PCN management team.
- Help prepare and disseminate internal communications, newsletters, and operational updates.
- Attend and contribute to PCN operational meetings and stakeholder forums.
- Build effective working relationships with external partners, ICB representatives, and neighbouring PCNs.
3. Workforce & Governance Support
- Assist in the implementation of workforce transformation initiatives, including support for ARRS roles and recruitment logistics.
- Maintain and support quality governance systems in collaboration with the wider team.
- Contribute to reporting, data tracking, and the maintenance of business continuity plans.
4. Data & Digital Enablement
- Work alongside the Digital & Transformation Lead to embed best practices in digital inclusion, data quality, and patient access improvement.
- Coordinate returns and data required for contractual compliance and local reporting.
