We are a dynamic, forward

About us
Primary
Care Networks (PCNs) form a key building block of the NHS long-term plan. General
Practices working together at scale has been a policy priority for some years
for a range of reasons. These include improving the ability of practices to
recruit and retain staff; to manage financial and estates pressures; to provide
a wider range of services to patients and to integrate with the wider health
and care system more easily.
Bradford
City 5 PCN is a large PCN with 10 practices and a patient list size of approximately
56,000 patients. We cover the geographical area of Bradford Moor, Barkerend, Thornbury

opportunities for in-house multi-disciplinary team working. We have a
well-developed team which includes a Pharmacy Team, Social Prescribers, Health
